APL General Laboratory Requisition 21302 (Alberta)

Alberta Precision Laboratories' provincial community requisition for routine blood work and common tests, from CBC and chemistry to serology, drug levels, urine tests, Framingham risk assessment and FIT. Community prescribers use it when ordering outside Connect Care; APL lists it, with Connect Care generated orders and specialty requisitions, as an approved way to order. The current revision is 21302 (Rev2026-09).

Who completes it

The authorizing provider, who orders the tests and acts on the results, completes the patient and provider blocks, copy-to recipients, tests and clinical information. The collector completes the collection date, time, location and collector ID.

Submitting and payment

How to submit it
Community clinics give the patient a paper copy, which the collecting laboratory site still requires; APL's Requisition Generator can produce a PDF pre-populated with provider details. Since October 1, 2025, Connect Care facilities no longer need to print paper requisitions in most cases.
Who pays
Tests are registered under the patient's PHN; tick Third Party Bill and name the client when a third party pays, and give a full address for patients paying for their own tests.

Good to know

  • Both the Connect Care Provider ID and the Submitter ID are required to route reports: the Provider ID follows the provider across locations, while the Submitter ID identifies the clinic.
  • APL needs the patient's full legal name plus at least one unique identifier, such as ULI or PHN; date of birth does not count, and out-of-province patients need their PHN expiry date.
  • A differential is not automatic with a CBC. APL warns against assuming it is, so tick CBC and Differential rather than CBC no Differential when the white cell differential matters.
  • Transfusion testing uses Transfusion Medicine Requisition 21448 and routine prenatal red cell screening uses the Canadian Blood Services requisition. Unlisted tests go under Additional Tests after checking the APL Test Directory.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Can patients request billing confidentiality on an APL requisition?

Yes. APL's guidance on completing requisitions says patients who want billing confidentiality can have 'Suppression of Claim' written in the identifier field, and the PHN is then left off. A phone number is still required for community patients, and an address is needed for out-of-province patients and those paying for their own tests.

What does the Framingham risk assessment on form 21302 need?

The Cardiovascular Disease Risk Assessment box includes a lipid panel and requires history: systolic blood pressure, plus yes or no answers for current tobacco use, treatment for high blood pressure, diabetes, chronic kidney disease, atherosclerosis, and a first-degree relative with cardiovascular disease (male under 55, female under 65).

What do the F and I symbols mean on the APL requisition?

F marks a fasting test, with a box for hours fasting, and I points to a patient instruction sheet. Fasting glucose and the 2-hour glucose tolerance tests carry both symbols. Timed 24-hour urine collections carry I and need the start and end dates and times and the total volume recorded.
