Manitoba MRI Request Form R130-80-04

Manitoba's requisition for MRI examinations; non-emergent requests are faxed to Diagnostic Imaging Central Intake, which sends each to an appropriate site with the shortest wait. Besides patient, scheduling and exam details, it includes MRI safety pre-screening for implants and metal, renal questions for IV contrast, and pediatric sedation prompts. Version V03 has been in effect since May 1, 2019.

Who completes it

The referring clinician completes and signs the form, including the asterisked patient, weight, height, allergy, exam and clinical history fields. The referring office attaches implant records and any orbital X-ray report.

Submitting and payment

How to submit it
Fax non-emergent requests to Diagnostic Imaging Central Intake, which distributes them to an appropriate location; for emergent requests, call the radiologist directly. Incomplete or illegible forms are returned.
Who pays
The ordering clinician block records a billing number, and the form asks whether other insurance applies, with a WCB claim number for compensation cases.

Good to know

  • Pre-appointment screening asks yes or no for pacemakers or defibrillators, neurostimulators, loop recorders, STRATA valves, cochlear implants, infusion pumps or glucose monitors, surgical implants and retained metal.
  • Patients who weld, work with metal or have metal in the eyes need an orbital X-ray report sent as soon as possible, and implant records should be attached to the requisition.
  • Pediatric patients under 10 who need sedation require the pre-sedation history and physical form, and children 2 years or younger need a head circumference percentile.
  • For IV contrast exams, flag renal disease (dialysis, transplant, single kidney, kidney surgery or cancer involving the kidneys) with the latest serum creatinine, eGFR and date, and note any PICC, CVC or port.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Can I request a specific MRI site in Manitoba?

Yes. The outpatient section offers the first available appointment in Winnipeg, willingness to travel within Manitoba for the first available slot, or a named preferred site. ED outpatient requests record the time the order was placed and whether follow-up is with the ED physician or the primary care provider.

Why would an MRI requisition be returned in Manitoba?

The form states that incomplete or illegible forms will be returned and that asterisked information must be completed or the request will be declined. Required items include patient name, date of birth, sex, PHIN, daytime phone, weight, height, imaging-related allergies, the anatomical location or exam, relevant clinical and surgical history, and the clinician's signature and printed name.

Is there a separate form for research MRI scans?

Yes. The form directs research study requests to the Research Requisition Form rather than R130-80-04. Shared Health also publishes an MRI Requisition Reference Guide showing the mandatory fields, and an MRI Safety Checklist, on its diagnostic imaging central intake page.
