Manitoba Lab Requisition R250-10-94 (Winnipeg/Brandon)

Shared Health's standardized requisition for outpatient biochemistry, hematology and therapeutic drug monitoring ordered from clinics and community settings in Winnipeg and Brandon. It covers electrolytes, renal and liver tests, lipids, A1c, TSH reflex, iron studies, CBC, coagulation, malaria and drug levels. Version V02, implemented March 20, 2024, added a fasting-instruction section; clinics outside the two cities use R250-10-92.

Who completes it

The ordering provider completes the provider, copy-to, patient and fasting sections and ticks the tests, with mandatory fields marked by an asterisk. The person collecting the sample completes the collection fields marked with a diamond.

Submitting and payment

How to submit it
The printed requisition goes with the patient or specimen to the collecting laboratory. When the fillable PDF is used, the form says it must be completed in one sitting and printed immediately, without saving personal health information on the computer.
Who pays
The provider block includes a billing code, and the patient's PHIN identifies coverage, with another province or DND specified if different; the form lists no patient charges.

Good to know

  • Mandatory fields include the ordering provider's last and full first name, a critical results phone number and facility address, and the patient's name as on the health card, date of birth, sex and PHIN.
  • Shared Health makes the ordering practitioner of record legally responsible for critical results, so outpatient requisitions need an after-hours contact number unless one is already in the laboratory system.
  • The fasting box applies to tests marked with a section sign, such as glucose, lipid panel, triglycerides and %TSAT; if nothing is checked, the lab assumes fasting is not required.
  • Oral glucose tolerance and glucose challenge tests were removed in 2024 and belong on the General Endocrinology requisition. ESR cannot be ordered with CRP unless approved.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What happens if the copy-to section of R250-10-94 is incomplete?

The form warns that if copy-to information is missing, the report may not be sent to that recipient. Give the full last and first name, phone, fax and facility name or address for each copy. Missing or illegible mandatory fields elsewhere on the form can result in specimen rejection.

What dose information do drug levels need on R250-10-94?

The therapeutic drug monitoring section, covering drugs such as lithium, digoxin, vancomycin trough, tacrolimus and valproic acid, states that dose information must be completed: the last dose date and time and the next dose date and time. For PT/INR, the form also asks whether the patient is on an anticoagulant and which one.

Can a clinic modify Manitoba's standardized lab requisitions?

No. Shared Health's 2021 practice change states the requisitions are owned and document-controlled by Shared Health Diagnostic Services and cannot be altered for site-specific use without its approval. EMR vendors and print shops receive the official versions, and prior versions should be deleted from the EMR when a new form replaces them.
