Alberta Form 1: Admission Certificate (Mental Health)

A physician, nurse practitioner or other qualified health professional issues this certificate within 24 hours of examining someone who meets all four admission criteria of Alberta's Mental Health Act. One certificate authorizes conveyance to a designated facility and 24 hours of detention there for a second examination. A second Form 1 within those 24 hours makes the person a formal patient for one month.

Who completes it

The examining qualified health professional completes the whole certificate, ticking whether they are a physician or another qualified health professional. Of the two certificates needed for formal patient status, section 7 requires at least one to be issued by a physician and at least one by a member of the facility's staff.

Submitting and payment

How to submit it
If conveyance is ticked, the certificate travels with the person to the named facility, where the board reviews it for completeness, issues it and gives it to the patient. AHS sites can also complete it as a Connect Care eForm.

Good to know

  • Section 4 of the Act limits the authority to apprehend and convey under one certificate to 72 hours from the time it is issued.
  • The four criteria: mental disorder, potential to benefit from treatment, likely harm to others or negative effects such as substantial deterioration within a reasonable time, and unsuitability for admission except as a formal patient.
  • For each criterion the form asks the examiner to tick whether the facts were observed by them, communicated by others, or both, as section 6 of the Act requires.
  • The examination can be in person or by video conference; the form records which, with the place of examination or the person's location.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a nurse practitioner issue an Alberta Form 1?

Yes. Section 2 of the Mental Health Act lets any qualified health professional issue an admission certificate, and the Act defines that term to include a physician or nurse practitioner. For formal patient status, though, at least one of the two certificates must come from a physician and at least one from a member of the facility's staff.

How long is an Alberta admission certificate valid?

It must be issued within 24 hours of the examination. One certificate allows conveyance to a facility within 72 hours of issue and detention for 24 hours after arrival. If a second certificate is not issued in that time, the person is released or may stay voluntarily; two certificates authorize detention for one month from the second.

What does the AHS completeness review check on Form 1?

It checks the issuer's name, business address and type, the person's name, the examination date, time, means and location, facts for all four criteria with their source marked, the conveyance facility when issued outside a designated facility, the issue date and time, and the signature. Gaps go to the most responsible qualified health professional.
