Mental Health Act forms for involuntary assessment and admission

The certificates behind involuntary psychiatric assessment, admission, renewal and community treatment, 51 forms across Canada, with who signs each and how long it lasts.

Each province and territory prescribes its own forms for involuntary psychiatric care, and most begin with a clinician's certificate. In Ontario it is Form 1, the Application by Physician for Psychiatric Assessment, which allows detention in a psychiatric facility for up to 72 hours. British Columbia splits admission between Form 4.1 and Form 4.2, the first and second medical certificates that took effect in 2023 and replaced the single Form 4. Alberta's Form 1 Admission Certificate, Saskatchewan's Form A, Manitoba's Form 4, Nova Scotia's Form 2 and Newfoundland and Labrador's MHCTA-01 do the same job under their own rules.

Time limits and signers differ. A BC first certificate stays valid for up to 14 days, and the second must follow within 48 hours of admission. Nova Scotia's Form 2 is not effective unless signed within 72 hours of the examination, and New Brunswick's Form 1 within seven days. A nurse practitioner can sign the first certificate in BC, PEI and Newfoundland and Labrador. Manitoba's online forms are for information only: legal copies come from the Chief Provincial Psychiatrist's office. Renewal and community treatment order forms are listed for each province too.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What is a Form 1 under Ontario's Mental Health Act?

The Application by Physician for Psychiatric Assessment (6427-41). A physician who personally examined the person within the past seven days completes Box A, the serious harm test, or Box B, for people incapable of consenting to treatment who meet its criteria. It authorizes apprehension within 7 days and detention in a psychiatric facility for up to 72 hours, and the person must promptly be given a Form 42.

What replaced Form 4 in British Columbia?

Forms 4.1 and 4.2 took effect on February 1, 2023, and the old Form 4 was repealed on February 1, 2024. A physician or nurse practitioner completes Form 4.1, which is valid for up to 14 days and supports admission for up to 48 hours. A different physician completes Form 4.2 within those 48 hours to continue admission for up to one month.

Can a nurse practitioner sign a Mental Health Act certificate?

In some provinces. BC's Form 4.1, PEI's Form 1 and the first certificate on Newfoundland and Labrador's MHCTA-01 can be signed by a physician or nurse practitioner, and Alberta's Form 1 by a physician, nurse practitioner or other qualified health professional. In the Northwest Territories, Form 2 can come from a medical practitioner, nurse practitioner, psychologist or registered nurse. Ontario's Form 1 is a physician's application.

How long does an involuntary psychiatric assessment hold last?

Usually up to 72 hours. Ontario's Form 1, Manitoba's Form 4, New Brunswick's Form 1 and the first certificate on Newfoundland and Labrador's MHCTA-01 each allow up to 72 hours for assessment, and Nova Scotia's hold starts with the second certificate. Alberta's first certificate allows 24 hours at the facility for a second examination, and BC's Form 4.1 admission lasts up to 48 hours.
