PEI Mental Health Act Form 4: Renewal Certificate

The attending psychiatrist uses this certificate under section 11 of PEI's Mental Health Act to continue a patient's involuntary admission. It follows a new psychiatric assessment within the previous 72 hours. The renewal period depends on how many renewals came before: 30 days for the first, 90 days for the second or third, and 12 months for the fourth and later ones.

Who completes it

The attending psychiatrist completes and signs the whole certificate, recording the assessment times, the three opinions, the facts and observations behind them, information from other sources, and which renewal this is.

Submitting and payment

How to submit it
The certificate is completed at the psychiatric facility where the patient is held. There is no external submission step printed on the form; it is issued before the current certificate expires.

Good to know

  • The criteria match Form 3: a mental disorder, harm or likely harm or substantial deterioration as a result, and a need for care and treatment in a psychiatric facility.
  • The psychiatrist confirms again that the patient refuses or is unable to consent to admission to a psychiatric facility.
  • Each renewal expires on its own date unless the attending psychiatrist cancels it sooner, so the checkbox for first, second or third, or fourth or later renewal matters.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long is a renewal under PEI's Mental Health Act?

A first certificate of renewal expires 30 days from the date it is issued. A second or third renewal lasts 90 days, and a fourth or later renewal lasts 12 months. Any certificate can be cancelled sooner by the attending psychiatrist.

What assessment does a PEI renewal certificate need?

The attending psychiatrist must have personally conducted a psychiatric assessment of the patient within the previous 72 hours, at the psychiatric facility, and record when it started and ended. The certificate also asks for the facts and personal observations behind the opinion, kept apart from information from other sources.
