PEI Mental Health Act Form 3: Involuntary Admission

After assessing someone at a psychiatric facility, a psychiatrist uses this certificate under section 10 of PEI's Mental Health Act to admit them as an involuntary patient. The psychiatrist must have personally assessed the person within the previous 72 hours. The certificate expires 30 days after it is issued unless the attending psychiatrist cancels it sooner.

Who completes it

The psychiatrist who conducted the assessment completes the whole certificate, recording the facility and the start and end times of the assessment, the opinion boxes, the facts behind the opinion, information from other sources, and the date of issue and signature.

Submitting and payment

How to submit it
The certificate is issued at the psychiatric facility where the person is admitted, one of Hillsborough Hospital, Prince County Hospital or the Queen Elizabeth Hospital. There is no external submission step printed on the form.

Good to know

  • Three opinions are required: a mental disorder, harm or likely harm to the person or others or likely substantial deterioration as a result, and a need for care and treatment in a psychiatric facility.
  • The psychiatrist must confirm that the person refuses or is unable to consent to admission to a psychiatric facility.
  • Facts and personal observations are written separately from information received from other sources, and those sources must be identified.
  • To continue involuntary admission past 30 days, the attending psychiatrist completes Form 4, the Certificate of Renewal of Involuntary Admission.

Frequently asked questions

How long does a PEI Certificate of Involuntary Admission last?

Thirty days from the date it is issued, unless the attending psychiatrist cancels it sooner. Continuing the admission after that requires a Certificate of Renewal (Form 4), which lasts 30 days for a first renewal, 90 days for a second or third, and 12 months for a fourth or later renewal.

Who can complete Form 3 in PEI?

A psychiatrist who has personally conducted a psychiatric assessment of the person at the psychiatric facility within the previous 72 hours. The certificate records when that assessment started and ended, and the psychiatrist confirms having made careful inquiry into the facts about the person's mental condition.
