PEI Mental Health Act Form 1: Psychiatric Assessment

A medical practitioner or nurse practitioner who has personally examined someone uses this order under section 8 of PEI's Mental Health Act to refer them for an involuntary psychiatric assessment. It is enough authority for a peace officer to take the person to a psychiatric facility, and for a psychiatrist to detain, restrain and observe them there for not more than 72 hours while assessing them.

Who completes it

The examining medical practitioner or nurse practitioner completes the whole order: their name and designation, when and where they examined the person, the opinion boxes and the supporting information, then the date, time and signature. No one else completes a part of it.

Submitting and payment

How to submit it
The signed order goes with the person to one of the psychiatric facilities designated in the regulations: Hillsborough Hospital, Prince County Hospital or the Queen Elizabeth Hospital. A peace officer may use it to apprehend the person and take them there.

Good to know

  • The practitioner must give the opinion that the person has a mental disorder and, as a result, has caused or is likely to cause harm to themselves or others, or is likely to suffer substantial deterioration.
  • The order also asks the practitioner to confirm that the person refuses or is unable to consent to a psychiatric assessment.
  • Form 2 orders the same assessment for a voluntary patient who asks to be discharged, and only the attending psychiatrist can issue it.
  • If the psychiatrist finds the admission criteria are met after the assessment, they complete Form 3, the Certificate of Involuntary Admission.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign a Form 1 in PEI?

A medical practitioner or a nurse practitioner who personally examined the person. The order records the practitioner's name and designation, the date, time and health facility of the examination, the opinion they reached, and supporting information such as details of the examination and the observations behind the opinion.

How long can someone be held under a PEI Form 1?

The order lets a psychiatrist detain, restrain and observe the person in a psychiatric facility for not more than 72 hours and conduct the involuntary psychiatric assessment. Keeping the person as an involuntary patient after that needs a psychiatrist's Certificate of Involuntary Admission, Form 3, which lasts 30 days unless cancelled sooner.
