SK Form G: Involuntary Admission Certificate

Physicians with admitting privileges to a mental health centre use this certificate to admit or hold someone involuntarily under section 24 of The Mental Health Services Act. Each certifying physician records an examination within the preceding 72 hours and the grounds for three opinions. Two certificates, at least one by a psychiatrist, authorize detention until the end of the 21st day; the same form is used to renew detention.

Who completes it

Each examining physician completes and signs their own certificate in front of a witness, after examining the person separately from any other practitioner. A supervised resident in psychiatry may issue one certificate, but the other must not come from that resident's supervisor or another resident.

Submitting and payment

How to submit it
The certificate's distribution line lists the mental health centre, the official representative and the officer in charge. Once the person is detained, they, their nearest relative and any proxy or personal guardian must also be informed of the admission and their rights.

Good to know

  • If two certificates are not reasonably practicable, one certificate allows detention until the end of the third day, and a second opinion must be obtained as soon as practicable.
  • For someone not already an in-patient, admission must happen within seven days of the first examination on which the certificates are based.
  • Detention continues for successive 21-day periods only on two new certificates, one by a psychiatrist, signed before the period ends; mandatory review panel appeals follow the first 21 days and every six months.
  • Form G requires a finding that the person cannot fully understand and make an informed decision about treatment, a criterion that Form A does not include.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How many Form G certificates are needed in Saskatchewan?

Two, from physicians with admitting privileges to a mental health centre, at least one of whom is a psychiatrist, each examining the person separately. If two are not reasonably practicable, one certificate allows detention until the end of the third day following admission, and a second opinion must be obtained as soon as practicable.

How long does a Form G admission last?

Two certificates authorize detention until the end of the 21st day following admission, or following the first certificate if the person is already an in-patient. Renewal needs two new Form G certificates signed before that period ends. Each certificate must state that the examination took place within the preceding 72 hours.

What criteria does Form G require?

The physician must have reasonable grounds to believe the person has a mental disorder needing treatment or care and supervision available only in a mental health centre, cannot fully understand and make an informed decision about that need, and is likely to cause harm to self or others or suffer substantial mental or physical deterioration if not detained.
