SK Form H.4: Certificate in Support of a CTO

A second psychiatrist completes this certificate after independently examining a person named in a new community treatment order. It confirms the six criteria are met, that the order satisfies the Act's requirements, and that the psychiatrist concurs with the treatment and services set out in the Form H.3. The certificate validates the order, which then runs for the period the issuing psychiatrist specified.

Who completes it

A psychiatrist other than the one who issued the order examines the person within the preceding 72 hours and signs the certificate. If no other psychiatrist is reasonably available in the region, a physician designated by the regional director under section 24.31 may complete it.

Submitting and payment

How to submit it
The Guide says the issuing psychiatrist sends a copy of Form H.3 to the second psychiatrist; once the order is validated, copies go to the patient, nearest relative, any proxy or personal guardian and the official representative.

Good to know

  • A certificate from a designated physician validates the order for only 72 hours from its date; without a psychiatrist's certificate in that time the order lapses and the process must be repeated.
  • Section 24.4(4) says no supporting certificate is needed for later renewals as long as the order has not lapsed.
  • If the person refuses the validating examination, the issuing psychiatrist may order them to submit to it and, if they still refuse, have them apprehended and conveyed for the examination.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete Saskatchewan Form H.4?

A psychiatrist other than the psychiatrist who issued the community treatment order. Where no other psychiatrist is reasonably available in the region, a regional director may designate a physician, but that physician's certificate validates the order for only 72 hours, so a psychiatrist's certificate is needed to continue it.

Is Form H.4 needed to renew a CTO?

No, if there is no lapse. Section 24.4(4) of The Mental Health Services Act says a certificate in support is not required for any later renewal of a valid order, so renewals need one psychiatrist. If the order expires before it is renewed, the Guide says two new certificates are required.
