SK Form H.3: Community Treatment Order

A psychiatrist who has examined someone within the preceding 72 hours issues this order under section 24.3 of The Mental Health Services Act to require treatment, care and supervision in the community. It gives the grounds for six criteria, the services and treatment to be provided, the people authorized to make sure they are delivered, any required residence and a period of up to six months. A Form H.4 validates a new order.

Who completes it

The examining psychiatrist completes and signs the order in front of a witness and ticks New or Renewal. Page 2 names the person authorized by the regional director to ensure the services are provided.

Submitting and payment

How to submit it
Once the order is validated, the attending physician gives copies to the patient, nearest relative, any proxy, any personal guardian and the official representative for the region, matching the distribution list printed on the form.

Good to know

  • The person must have been admitted to a mental health centre at least once in the preceding two years, voluntarily or involuntarily, or have been on a community treatment order before.
  • Renewals need only one psychiatrist and no Form H.4 if the order has not lapsed; an expired order needs two new certificates.
  • The order may require the person to stay at a specified residence; the Guide says the psychiatrist can change that address without writing a new order.
  • If the person stops complying and refuses examination, the attending physician or a prescribed health professional may order apprehension on Form H.7.

How this list is checked

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long does a Saskatchewan CTO last?

The psychiatrist sets the period on page 2, up to six months from the date of the order when it is validated by a psychiatrist. If the supporting certificate comes from a physician designated by the regional director because no second psychiatrist is available, the order lasts only 72 hours unless a psychiatrist issues a further certificate.

Who qualifies for a community treatment order in Saskatchewan?

Someone with a mental disorder needing treatment or care and supervision in the community who, in the past two years, was admitted to a mental health centre or was on a prior order; is likely to cause harm or deteriorate without services; has those services available; cannot fully understand the need for treatment; and can comply with the order.
