Yukon Form 11: Renewal of Involuntary Admission

This Mental Health Act certificate extends a Yukon patient's involuntary admission beyond the current certificate. Shortly before a Form 7 certificate or an earlier renewal expires, the attending physician and one other physician each examine the patient and complete a separate copy, recording the original admission date and facility, current findings, a diagnosis and the grounds for continued involuntary care.

Who completes it

The attending physician and one other physician each complete and sign a separate copy of the whole form, with a witness signature. Nothing on the certificate is completed by the patient.

Submitting and payment

How to submit it
Both copies go to the Capability and Consent Board and the Chief Executive Officer of Whitehorse General Hospital, filed immediately under section 16 of the Act. The patient receives both copies with blank Forms 8 and 9, and the nearest relative, proxy or guardian receives them with a blank Form 9.

Good to know

  • Renewal requires the section 13(1) admission prerequisites: a mental disorder likely to cause serious bodily harm or impending serious impairment unless the patient stays in hospital custody, and unsuitability for voluntary admission.
  • Section 16 of the Act limits each certificate of renewal to a maximum of 21 days of detention, observation and examination.
  • If the physicians do not renew, the Act requires the attending physician to promptly tell the patient they may leave hospital unless lawfully detained for another reason.
  • Section 30 of the Act requires the Capability and Consent Board to review each renewal within seven days of receiving the certificates.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When should a Yukon Form 11 be completed?

Section 16 of the Mental Health Act requires the examination shortly before a certificate of involuntary admission or its renewal expires. An involuntary patient whose authorized period has expired is deemed a voluntary patient under section 17, so the renewal has to be completed and filed before the current period ends.

How long does a Yukon renewal certificate last?

Each certificate of renewal authorizes detention, observation and examination in hospital for not more than 21 days, under section 16(2) of the Act. A further renewal follows the same process: a new examination by the attending physician and one other physician, and two new copies of Form 11.

Is Form 11 available in French?

Yes. Yukon posts Formulaire 11, Certificat de renouvellement d'admission non volontaire, on its French forms page. Both versions are three-page fillable PDFs; the English edition is dated July 2019 and the French edition September 2019.
