Yukon Form 4: Recommendation for Psychiatric Assessment

A Yukon physician completes this Mental Health Act form after examining a person to assess their mental condition under sections 5 and 10 of the Act. The physician records the examination findings, any prior knowledge and behaviour reported by others, then chooses release or recommends involuntary psychiatric assessment at a named designated facility. A recommendation lets the person be taken to hospital for examination by two physicians.

Who completes it

The examining physician completes and signs the whole form, and a witness signs below. Yukon posts a separate form for a nurse who makes the same recommendation, and the Act requires that nurse to consult a physician.

Submitting and payment

How to submit it
If assessment is recommended, the form requires copies for the Chief Executive Officer of Whitehorse General Hospital, the patient, the nearest relative, proxy or guardian if available, and the Capability and Consent Board; if the patient is released, only the Board receives a copy. When travel is needed, an Application for Medical Travel goes to Whitehorse General Hospital with the form.

Good to know

  • Option C requires a mental disorder plus one of four grounds: threatened or attempted self-harm, violence toward another person, causing fear of bodily harm, or a recent lack of ability to care for oneself.
  • Each ground on the form pairs the behaviour with a likely outcome, such as likely bodily harm or likely impending serious physical impairment, and the physician must write the evidence supporting that opinion.
  • Section 12 of the Act requires two physicians to examine and assess the person separately within 24 hours of arrival at hospital; each uses Form 7 if admitting involuntarily.
  • Where the community has no hospital, the Act treats the recommendation as certification that the person needs care not available locally, for the purposes of the Travel for Medical Treatment Act.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long is a Yukon Form 4 recommendation valid?

Section 11 of the Mental Health Act makes the recommendation sufficient authority for a peace officer or other person to take the person into custody as soon as possible, and no later than the end of the sixth day after the day it was made. The person can then be detained and observed in hospital for up to 24 hours.

What does the physician record on Form 4?

The form asks for the affective, cognitive and behavioural presentation at examination, including appearance, speech, thought content, insight, judgment and diagnosis. Separate sections cover the physician's prior knowledge of the patient and behaviour observed by others, naming the source, the observer, the timing and what was seen. An admitting history can be appended.

Is Form 4 available in French?

Yes. Yukon posts a French version, Formulaire 4, Recommandation d'évaluation psychiatrique non volontaire (médecin), on its French forms page. Both versions are three-page fillable PDFs; the English edition is dated July 2019 and the French edition September 2019.
