Yukon Form 12: Certificate of Change of Status

A physician uses this one-page Mental Health Act certificate to end a Yukon patient's involuntary status. After examining the patient, the physician states that the person is not, or is no longer, suffering from a mental disorder likely to cause serious bodily harm or substantial mental or physical impairment without involuntary admission, and revokes the Form 7 or Form 11 certificates in effect.

Who completes it

The physician who examined the patient records the date and time of the examination, then dates and signs the certificate; section 17(2) of the Act assigns the change of status to the attending physician.

Submitting and payment

How to submit it
Copies go to the Capability and Consent Board, the Chief Executive Officer of Whitehorse General Hospital, the patient, and the next of kin, proxy or guardian if available. The Act requires filing with the person in charge of the hospital and the Board.

Good to know

  • Under section 17(2), the change applies when the involuntary admission prerequisites in section 13(1) are no longer met and the prerequisites for voluntary admission in section 13(2) are met.
  • The form has checkboxes for revoking Form 7, the Certificate of Involuntary Admission, and Form 11, the Certificate of Renewal of Involuntary Admission; the physician marks the certificates in effect.
  • After a change of status, the person in charge must tell the patient they are voluntary, obtain consent to further care under section 21, and explain their right to leave.
  • Form 12 is not needed when a certificate simply expires: section 17(1) deems an involuntary patient whose authorized detention period has ended to be a voluntary patient.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is a Yukon Form 12 used?

When the attending physician believes an involuntary patient no longer meets the section 13(1) criteria but does meet the criteria for voluntary admission, before the current certificate runs out. Completing and filing Form 12 revokes the Form 7 or Form 11 then in effect and changes the patient's status to voluntary.

Who receives copies of the Certificate of Change of Status?

The form lists four recipients: the Capability and Consent Board, the Chief Executive Officer of Whitehorse General Hospital, the patient, and the next of kin, proxy or guardian if one is available. Section 17(2) of the Act also requires filing with the person in charge of the hospital.

Is Form 12 available in French?

Yes. Yukon posts Formulaire 12, Certificat de changement de statut, on its French forms page alongside the English version. Both are one-page fillable PDFs; the English edition is dated July 2019 and the French edition September 2019.
