NL MHCTA-03: Community Treatment Order

A psychiatrist uses this nine-page Mental Health Care and Treatment Act form to issue a community treatment order in Newfoundland and Labrador, so a person can receive continuing treatment or care and supervision while living in the community. It contains the order, a notice of rights, a transfer-of-responsibility section and the community treatment plan; the order expires six months after it is issued unless renewed.

Who completes it

The issuing psychiatrist completes and signs Section 1 and is responsible for the order's general supervision and management. Treatment team members named in the plan sign to confirm their agreement, and both psychiatrists sign Section 3 if responsibility is transferred.

Submitting and payment

How to submit it
The original must go in the file; the copy line lists the patient, patient representative, rights advisor, administrator and each treatment plan member, matching the Act's requirement to give copies to the person, representative, rights advisor and each named provider.

Good to know

  • During the preceding two years, the person must have been detained as an involuntary patient on three or more separate occasions or been subject to a prior order; the form records the dates.
  • The psychiatrist must have examined the person within the preceding 72 hours and must also be of the opinion that the person is capable of complying with the order's requirements.
  • Each health professional, person or organization named in the plan must have been consulted and agreed in writing to be named; the order may be signed in counterparts.
  • Renewal is for another six months at any time before expiry, with no limit, and the first renewal and every second renewal after it bring a Review Board application.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long does an NL community treatment order last?

Section 47 of the Act says an order expires six months after it is made unless it is renewed, terminated or revoked. It can be renewed before expiry for another six months, any number of times, and a renewal follows the same steps as issuing the order.

What does the community treatment plan section cover?

Section 4 of the form names the person responsible for supervising the plan, the psychiatrist and alternate psychiatrist, treatment and medications, reporting obligations, drug coverage, income, housing, community supports and a crisis plan, and states the person's obligation to attend appointments and take prescribed treatment.

When does the rights advisor meet the person?

The notice printed in Section 2 says a rights advisor meets the person, or their representative, within 24 hours of the order being issued and again within 10 days after that first meeting, and can help them apply to the Mental Health Care and Treatment Review Board.
