NL MHCTA-02: Certificate of Renewal (Involuntary)

When an involuntary patient's 30-day detention or an earlier renewal is about to end, the attending physician uses this one-page Mental Health Care and Treatment Act form to renew involuntary status in Newfoundland and Labrador. The physician ticks which renewal it is, from first to sixth or other, certifies the patient still meets the subsection 17(1) criteria, and sets out the facts and reasons.

Who completes it

The attending physician completes and signs the certificate, with the date and time, after conducting the psychiatric assessment required by subsection 30(1) of the Act. No one else completes a part.

Submitting and payment

How to submit it
Under subsection 30(3), the original goes in the patient's chart and a copy to the administrator, and the attending physician must also give the patient a copy. The copy line lists the original, patient, patient representative and administrator.

Good to know

  • Subsection 30(1) requires the assessment within the 72 hours immediately before the 30-day detention period expires; if the criteria are no longer met, the person is told involuntary status has ended.
  • Section 31 allows up to 30 days under the first renewal, 60 additional days under the second, and 90 additional days under the third and each later renewal, with no limit on renewals.
  • The administrator must apply to the Review Board for a review when a second certificate of renewal is filed and at each second renewal after that.
  • As on MHCTA-01, the facts the physician observed must be distinguished from those communicated by another person, and additional pages may be attached.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
