NB Form 34.VII: Supervised Community Care Renewal

A supervised community care plan in New Brunswick expires one year after approval unless renewed. To renew it, the psychiatrist personally examines the individual and uses this one-page application under section 34.08 to confirm the criteria are still met and that the plan has demonstrated effectiveness, asking the Review Board to renew it for one year unless cancelled.

Who completes it

The applying psychiatrist records the examination date and location, ticks each continuing criterion, describes how the criteria continue to be met and signs. The individual also signs and dates the form.

Submitting and payment

How to submit it
Section 34.08 requires the renewal application to be made to the review board at least 30 days before the plan expires, and it triggers a hearing within ten business days. The signed form is also sent immediately to Psychiatric Patient Advocate Services.
Who pays
Medicare service code 9386, MHA completion of other forms, applies under the April 2026 New Brunswick Physicians' Manual, limited to one per patient per day.

Good to know

  • Six criteria are ticked: serious mental illness, continuous illness, severely limited community functioning, need for care and treatment, lack of capacity to consent, and likely serious harm or substantial deterioration.
  • Beyond the criteria, the psychiatrist states the opinion that the plan has demonstrated effectiveness, and the form gives a single narrative box for how the criteria continue to be met.
  • A renewal may include amendments to the plan without the separate amendment notice process; amendments made outside renewal use Forms 34.III to 34.VI.
  • If a plan expires without renewal, section 34.03 requires the psychiatrist to make sure the person is notified that it has expired.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When must a supervised community care renewal be filed in New Brunswick?

Section 34.08 of the Mental Health Act requires the psychiatrist to apply to the review board, on the Minister's form, at least 30 days before the plan expires. The board must then hold a hearing within ten business days, which the chair may extend by another ten.

How long does a renewal last?

The form applies to renew the plan for one year, unless it is cancelled sooner. The psychiatrist must cancel the plan if the person no longer meets the criteria, and the review board may also cancel it during a hearing.

Is there a French renewal form?

Yes. Formule 34.VII, Soins communautaires supervisés, Formule de renouvellement, is a separate French PDF. Both versions carry a box recording whether the patient or client chose English or French for completion of the renewal.
