NB Form 34.I: Supervised Community Care Application

A psychiatrist uses this nine-page application to ask the Mental Health Act Review Board to approve a supervised community care plan for a person with a serious mental illness who lacks capacity to consent to treatment. It sets out the section 34.01 eligibility criteria, the planning meeting, each team member's role and obligations, and the required appointments, medications and housing. The board holds a hearing before the plan takes effect.

Who completes it

The applying psychiatrist, the attending psychiatrist for a hospitalized person or a community psychiatrist otherwise, completes the criteria notes and care plan and signs. The client signs an acknowledgement, and the person who reviewed the plan with the client also signs.

Submitting and payment

How to submit it
The application goes to the chair of the review board, which must hold a hearing within ten business days, extendable by ten more, and the psychiatrist must attend. The form also directs that it be sent immediately to Psychiatric Patient Advocate Services once signed.
Who pays
The April 2026 New Brunswick Physicians' Manual lists Medicare service code 9386 for completing Mental Health Act forms other than Form 1, at most once per patient per day.

Good to know

  • Eligibility requires a serious mental illness that is continuous, severely limits community functioning and requires care and treatment, plus prior admission or a harmful pattern of behaviour, and lack of capacity to consent.
  • The planning meeting section records whether the client attended and, if not, why and what efforts were made, along with the treating psychiatrist, team members, caregiver and nearest relative.
  • An approved plan lasts up to one year and the client may request one review in that year. A psychiatrist may issue a 30-day certificate of non-compliance on Form 34.VIII.
  • The client's signature acknowledges being told about the plan, the right to counsel and to Psychiatric Patient Advocate Services, and the consequences of non-compliance; it does not mean agreement.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can apply for supervised community care in New Brunswick?

A psychiatrist applies: the attending psychiatrist if the person is hospitalized, or a community psychiatrist if not. The psychiatrist must believe the plan is in the person's best interests and give reasons the section 34.01 criteria are met, including that the person lacks capacity to consent to treatment.

How long does a supervised community care plan last?

Once approved by the Mental Health Act Review Board, a plan lasts one year unless renewed or cancelled. The psychiatrist must cancel it if the person no longer meets the criteria, and renewal uses Form 34.VII, filed at least 30 days before the plan expires.

Is the SCC application available in French?

Yes. Formule 34.I, Demande de plan de soins communautaires supervisés, is posted as a separate French PDF with the same nine pages, and both versions record whether the patient or client chose English or French for completion.
