Nova Scotia IPTA Form 5: Involuntary Admission Renewal

Before a declaration of involuntary admission or a previous renewal expires, the attending psychiatrist examines the patient and uses this declaration to renew involuntary status in a Nova Scotia psychiatric facility. It identifies the original Form 4 and its expiry, numbers the renewal, restates all the section 17 criteria and confirms the written treatment plan has been prepared and shared.

Who completes it

The attending psychiatrist on the staff of the psychiatric facility completes and signs the whole declaration. A new Form 5 is completed for each renewal.

Submitting and payment

How to submit it
It is filed with the chief executive officer or designate through the designated Nova Scotia Health or IWK office managing IPTA health records, and also with the provincial IPTA Review Board administration when it is associated with a mandatory hearing.

Good to know

  • Section 22 sets renewal terms of up to 30 days for the first renewal, 60 days for the second and 90 days for the third and each later renewal.
  • A renewal runs from the expiry date on Form 4 or the previous renewal unless the psychiatrist specifies otherwise, according to the form's instructions.
  • Two checkboxes confirm that a written, individualized treatment plan was prepared within 30 days of admission under section 20A and that a copy was given to the patient and substitute decision-maker.
  • If involuntary status is not renewed, the attending psychiatrist completes Form 6, the Declaration of Change of Status, and the patient is told of the right to leave.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long can a Nova Scotia involuntary admission be renewed for?

Under section 22 of the Act, a first declaration of renewal allows up to 30 additional days, a second up to 60 additional days, and a third or later renewal up to 90 additional days each. Each renewal needs a fresh examination by the attending psychiatrist and a new Form 5.

When are mandatory Review Board hearings due?

Section 37 deems an application to the Review Board 60 days after the initial declaration, then at 180 days, one year, one year and 180 days, and two years, and yearly after that. The form says the facility must track status and tell IPTA administration when hearings are due.
