Alberta Form 2: Renewal Certificate (Mental Health)

Before a formal patient's current certificates run out, two qualified health professionals examine the patient separately and, if all four admission criteria are still met, each issues this renewal certificate within 24 hours of their own examination. Two renewal certificates extend detention for one additional month the first time, one more month the second time, and up to six additional months each time after that.

Who completes it

Each of the two examiners completes their own certificate, ticking psychiatrist or other qualified health professional. Section 8 requires at least one certificate to be issued by a psychiatrist and at least one examiner to be a member of the staff of the facility where the patient is detained.

Submitting and payment

How to submit it
Both certificates stay with the facility: the board reviews each for completeness, issues it and gives it to the patient, with copies to the guardian, one person the patient designates and the nearest relative unless the patient objects on reasonable grounds.

Good to know

  • Each examiner certifies that they examined the person separately from any other qualified health professional, and names the facility where the examination took place.
  • Once a patient has been under admission or renewal certificates for a continuous six months without a review application, section 39 deems an application to the review panel.
  • The board must also give the patient a written statement of the reasons for renewal, the detention period, review panel rights, legal counsel and how to reach the Mental Health Patient Advocate.
  • Issuing a community treatment order on Form 19 cancels the patient's admission and renewal certificates; Form 2.1 is the separate form for cancelling a certificate.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who must sign an Alberta renewal certificate?

Two qualified health professionals, each after a separate examination. Section 8 of the Mental Health Act requires at least one of them to be a psychiatrist and at least one to be on the staff of the facility where the patient is detained. The AHS completeness review flags a certificate that does not identify its issuer as a psychiatrist or other qualified health professional.

How long does an Alberta Form 2 renewal last?

Two renewal certificates authorize detention for not more than one additional month the first time, not more than one additional month the second time, and not more than six additional months the third and each later time. Each certificate must be issued within 24 hours after that examiner's own examination of the patient.
