Alberta Form 20: Renewal of Community Treatment Order

Before a community treatment order expires, two qualified health professionals examine the person separately and complete this form to renew it for another six months. Part I and Part II confirm the person continues to have a mental disorder, is currently subject to an order, would likely suffer harm or deterioration without continuing community treatment, and can comply. The treatment plan, supervision and consent are restated for the new period.

Who completes it

The issuing qualified health professional for the renewal completes the Written Statement and Part I, the second examiner completes Part II, and one of the two must be a psychiatrist. Providers sign the Part III treatment and care plan, the supervising professional signs Part IV, and the person or substitute decision-maker signs Part V.

Submitting and payment

How to submit it
Copies and the written statement go to the person, any substitute decision-maker, the nearest relative unless the person objects on reasonable grounds, one designated person and all named providers. At the second renewal and every second renewal after, the issuing professional also sends the renewal forms to the review panel.

Good to know

  • Renewal allows 7 days between the first examination and completion of all parts, instead of the 72 hours allowed when the order is first issued on Form 19.
  • A renewal can be done at any time before expiry, and there is no limit on the number of renewals while the criteria continue to be met.
  • Unlike Form 19, the renewal does not ask for historical admission grounds: Part I and Part II instead confirm the person is currently subject to a community treatment order.
  • If medication in Part III is stated by class, such as an antidepressant, Recovery Alberta notes that changes within that class do not need a Form 21 amendment.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When should an Alberta CTO be renewed?

At any time before it expires, which is six months after it was issued or last renewed. Section 9.3 of the Act allows renewal only before expiry, so an order that has lapsed would have to be issued again on Form 19 with its full criteria. All parts of Form 20 must be completed within 7 days of the first examination.

Does a CTO renewal trigger a review panel hearing?

Yes, at the second renewal and every second renewal after that. Section 39(2) deems the person to have applied for a hearing on cancellation unless an application was made in the month before that renewal, and Recovery Alberta's flowchart asks the issuing professional to send the review panel a copy of the renewal forms.
