Manitoba MHA Form 7: Renewal Certificate

Shortly before an involuntary admission certificate or earlier renewal expires, the attending psychiatrist examines the patient and uses this Manitoba Mental Health Act certificate to continue involuntary status if the admission requirements are still met. The form records which renewal this is (first, second and so on), the date it takes effect, the current diagnosis and the facts relied on.

Who completes it

The attending psychiatrist of the involuntary patient completes and signs the whole certificate after personally examining the patient on the dates recorded.

Submitting and payment

How to submit it
The renewal is filed with the medical director, who must promptly check it complies with the Act. The printed copy line sends the original to the medical director and copies to the chart, the administrator of the Mental Health Review Board and the Director of Psychiatric Services.

Good to know

  • Each renewal certificate authorizes continued detention, restraint, observation, examination and treatment for not more than three months from its date, under subsection 21(4) of the Act.
  • The form restates the admission criteria in the present tense: the patient continues to have a mental disorder, still needs treatment only a facility can reasonably provide and cannot be admitted voluntarily.
  • On the filing of the third renewal certificate, and yearly after that while involuntary status continues, the patient is deemed to have applied to the Mental Health Review Board.
  • If the psychiatrist does not renew, they must promptly tell the patient they are now a voluntary patient; a patient whose certificate simply expires is deemed voluntary.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
