Alberta Disability Parking Placard Form (REG3164)

Albertans who cannot walk more than 50 metres, or whose vision loss substantially limits safe, independent mobility in parking areas, apply for a disability parking placard with this form. A healthcare professional certifies eligibility for a temporary placard of 3 to 12 months, a long-term placard of 5 years, or a permanent placard. The applicant then takes it to a registry agent.

Who completes it

A physician, surgeon, nurse practitioner, occupational therapist, physiotherapist or physical therapist, podiatrist or chiropractor completes the healthcare professional certification. The applicant completes their own details, and someone applying on another person's behalf needs a consent form from a registry agent.

Submitting and payment

How to submit it
The applicant brings the completed form and acceptable identification to an authorized Alberta registry agent within 6 months of the healthcare professional's signature.

Good to know

  • Permanent placards are renewed every 5 years by the client without a new application or a new medical certification.
  • A form signed more than 6 months earlier cannot be submitted, since the application must reach a registry agent within 6 months of the professional's signature.
  • Visitors can use a valid placard from their home province, territory or country, and short-term visitor placards must be returned to a registry agent before leaving Alberta.
  • The vision criterion covers loss of vision that substantially limits safe and independent mobility while navigating parking areas, not only limited walking distance.

How long does an Alberta disability placard last?

Temporary placards are issued for 3 to 12 months, long-term placards for 5 years, and permanent placards are renewed every 5 years by the client without a new application. Disabled licence plates are a separate option that carries a yearly registration fee.

Not to the government directly. The applicant takes the signed form and acceptable identification to an authorized registry agent office within 6 months of the healthcare professional's signature, and anyone applying for someone else also brings a registry agent consent form.
