SPARC BC Parking Permit Application: Doctor's Sections

SPARC BC, a non-profit, runs British Columbia's accessible parking permit program for people with mobility limitations, and every application needs a doctor's referral. The patient fills in their details and brings the form to their physician, who confirms the mobility-related disability, ticks the criteria met and recommends a permanent or temporary permit. SPARC BC then processes the application in about two to three weeks.

Who completes it

The applicant, or their power of attorney or legal guardian, completes Sections 1, 2, 6 and 8. The physician completes Sections 3 and 4 (assessment, eligibility criteria and permit recommendation) and Section 5 (name, phone, MSP number, address or stamp, signature and date).

How to fill it in

1. Applicant Information and 2. Previous SPARC BC Permits?

The applicant prints their name, mailing address, phone number, gender and date of birth, and can add an email address to renew online in the future. Section 2 asks whether they have held a SPARC BC permit before and, if so, the previous permit number. These steps are labelled for the applicant to complete before the form reaches you.

3. Physician Assessment and Confirmation of Eligibility

Write the patient's name and answer whether they have a mobility-related disability. Then check every criterion that applies: a disability that affects their mobility and specifically their ability to walk; unable to walk 100 metres without risk to their health; requires a mobility aid (wheelchair, walker, scooter or cane) to travel any distance; or other, including safety concerns, explained in the space provided. SPARC BC's FAQ adds that people who are blind or Deaf, or for whom safety is a consideration, can also be eligible.

4. Physician Recommendation

Choose one permit type: Permanent (renewable every three years); Permanent, but condition may improve (not renewable); or Temporary (not renewable, maximum one year). For a temporary permit, tick 1, 3, 6, 9 or 12 months and write the expiry date. SPARC BC's rules leave the duration of a temporary permit to the physician, and permits issued for permanent disabilities must be renewed every three years.

5. Physician Contact Information and Signature

Print your name, phone number and MSP number, and add your address or office stamp. By signing and dating, you certify that, for the reasons given, the patient has a mobility impairment that poses a risk to their health by walking 100 metres, and that the information is true and correct to your knowledge. The applicant's consent lets SPARC BC contact you to verify the disability and eligibility.

6. Payment Information

The applicant pays SPARC BC's processing fee by cheque, money order, Visa or Mastercard, including Visa and Mastercard debit cards; cash and regular debit are not accepted. The form states that any payment to the physician is separate from this fee. SPARC BC offers hardship assistance with the fee on request, and applicants may add a donation.

7. Rules of Use and 8. Signature and Declaration

The applicant reads the conditions: every application needs a referral from their doctor, one permit per applicant, permanent permits renew every three years, temporary permits last at most 12 months, and SPARC BC can rescind a permit for inaccurate information or misuse. The applicant, or their power of attorney or legal guardian with proof attached, signs and dates the declaration that the permit is for their own personal use and is not transferable.

Common mistakes

  • Leaving out physician details: the form requires your name, phone number, MSP number, contact information and address, plus your signature and the date.
  • Ticking Other without explaining it; the form asks you to explain other reasons, including safety concerns, in the space provided.
  • Recommending a temporary permit without choosing a duration and expiry date, or for longer than the 12-month maximum.
  • Assuming a temporary or 'condition may improve' permit can be extended: neither is renewable, so the patient will need a new application completed by the doctor.

Submitting and payment

How to submit it
The completed form goes back to the applicant, who returns it to SPARC BC by mail, in person or as a scanned email attachment. Processing takes about two to three weeks from receipt.
Who pays
Any charge for completing the physician sections is between you and the patient, and the form states that payments to the physician are separate from the processing fee the applicant pays SPARC BC.

Good to know

  • Permanent permits are valid for three years and renewable; temporary and 'condition may improve' permits are not, so another permit needs a new application completed by the doctor.
  • By signing, the applicant authorizes SPARC BC to contact their doctor to verify the disability and eligibility, and authorizes the doctor to release that information.
  • The permit is recognized across Canada and in several other countries, including the United States, and people qualify even if they do not own a vehicle.
  • Organizations that regularly transport people with mobility impairments may qualify for a permit, and permit holders who ride a motorcycle receive a decal instead.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Who can sign a SPARC BC parking permit application?

The medical sections are written for the applicant's doctor: Sections 3 to 5 must be completed by the physician, who certifies the mobility impairment and gives an MSP number, phone number and address. SPARC BC's FAQ describes the requirement as a referral from a medical professional with a valid MSP number.

What are the eligibility criteria for a SPARC BC parking permit?

The physician ticks the criteria that apply: a disability affecting mobility and the ability to walk, being unable to walk 100 metres without risk to health, needing a wheelchair, walker, scooter or cane to travel any distance, or other reasons including safety concerns, which must be explained. SPARC BC's FAQ also mentions people who are blind or Deaf.

How long does a SPARC BC parking permit take?

SPARC BC says processing takes two to three weeks from when the application is received. The form's sidebar reminds physicians that their name, phone number, MSP number, contact information and address, signature and date must all be included.

How long can a temporary SPARC BC permit last?

Up to 12 months. The physician ticks 1, 3, 6, 9 or 12 months and writes the expiry date in Section 4. Temporary permits cannot be renewed, so if the patient still needs one when it expires, the doctor completes a new application form.
