Applicants on BC's Housing Registry for subsidized housing can add this optional form when they are homeless, fleeing violence or abuse, or living with a serious health condition affected by their current housing. A third-party verifier describes the situation, and some housing providers give those applicants additional consideration. The Registry uses it to assess eligibility, housing need and which developments suit the household.
The applicant completes and signs Part One, then forwards the form to a verifier who completes Part Two: Section A for everyone, Section D for a health condition or disability, and the signed statement in Section E. For health conditions, accepted verifiers include physicians, registered or psychiatric nurses, occupational therapists and registered social workers.

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

BC Housing lists registered social workers, physicians, occupational therapists and registered or psychiatric nurses for serious health conditions. The verifier must be familiar with the applicant's situation, and other professionals may be considered case by case. Landlords, relatives, friends and neighbours cannot verify.
No. The form says some housing providers that fill units through The Housing Registry may give additional consideration to verified applicants. BC Housing's page notes that providers decide how to use it, and some non-profit and co-operative providers use other selection methods.
No. REG-005 is a 9-page form to print or type clearly. Its Section E statement confirms the verifier is not a relative or landlord, says how long they have known the applicant, and acknowledges the information will be shared with Registry housing providers.
