SAAQ Accessible Parking Permit (Vignette) Form

Quebecers apply to the SAAQ for an accessible parking permit, for a car or for a motorcycle, moped or scooter, with this four-page form. Section C screens out cases that need no assessment, such as a licence bearing condition P. Otherwise a listed professional completes Section D on diagnoses, impairments, permanence and outdoor mobility, and the SAAQ mails its decision in four to six weeks.

Who completes it

The person with a disability or their representative completes Sections A to C and signs Section F. When Section C requires an assessment, a professional listed in Section E, such as a physician, nurse, occupational therapist, physiotherapist, optometrist, psychologist, chiropractor, respiratory therapist, podiatrist or social worker, completes Sections D and E.

Submitting and payment

How to submit it
The applicant mails the form with a cheque or money order payable to the SAAQ. The decision arrives by mail in four to six weeks and is also posted in the applicant's SAAQclic account.
Who pays
The SAAQ does not reimburse fees the assessor may charge, so any charge for Section D falls to the applicant.

Good to know

  • No assessment is needed if the licence bears condition P, the SAAQ funded certain vehicle adaptations, or the RAMQ paid for a wheelchair, positioning base, adapted stroller or orthomobile.
  • Section D records how far the person can travel outdoors, unassisted and with aids, as 50 metres or less or more, and whether going outdoors risks illness or trauma.
  • Information on the form may also be used to assess the person's fitness to drive, as the form warns at the top of page 1.
  • A motorcycle, moped or scooter permit requires the applicant to own or lease that vehicle, and the parking permit itself is not refundable.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete the assessment on the SAAQ parking permit form?

Section E lists 14 occupational codes: physician, nurse, occupational therapist, optometrist, physiotherapist, physiotherapy technologist, psychologist, chiropractor, respiratory therapist, podiatrist, psychoeducator, special educator in the health and social services network, guidance and mobility specialist, and social worker. The assessor gives a licence number and dates the assessment and report.

Is the SAAQ accessible parking permit form available in French?

Yes. The original form is French, Demande de vignette de stationnement pour personnes handicapées (6375 30), and the SAAQ also publishes the English Accessible Parking Permit Application (6375A 30), which notes that the original document is in French.

Does every applicant need a health professional's assessment?

No. Section C screens for cases that need none: a driver's licence bearing condition P, SAAQ vehicle adaptation funding for items such as a lowered floor or hoist, or a RAMQ-paid wheelchair, positioning base, adapted stroller or orthomobile. Otherwise, Section D must be completed.
