SAAQ 4052: Résumé d'évaluation initiale, psychologie

After a road-accident victim's first psychological assessment, the psychologist or psychotherapist sends this summary to the SAAQ. It gets the assessment reimbursed and authorizes balise 1, up to 15 treatment hours for psychological disorders accepted as linked to the accident. The six-page form covers the assessment, its conclusion and an intervention plan, and the 13-page package adds an invoice, a guide and writing prompts.

Who completes it

The psychologist or psychotherapist who did the assessment completes Sections 1 to 4, notes whether the summary was discussed with the accident victim, and signs it. Psychologists give diagnoses and personality findings, while psychotherapists summarize their clinical observations.

Submitting and payment

How to submit it
Fax the summary with its receipt or invoice (4059 30) to the SAAQ after the initial assessment; the original is not required, and mail is also accepted. Filling it on screen carries the person's name and claim number onto the invoice.
Who pays
The SAAQ reimburses up to three hours for doing the assessment and up to two hours for completing this form, paid through the invoice or reimbursed to the victim using the receipt.

Good to know

  • Psychologists state each diagnosis's severity (mild, moderate or severe) and whether it is provisional or confirmed; a conclusion on personality traits or disorders can be deferred.
  • The problem history is not a full anamnesis; include pre-accident symptoms that returned or worsened after the accident, and add history later in follow-up if needed.
  • For each therapy goal, name the techniques used, such as in vivo driving exposure or cognitive restructuring, how results will be measured and the target date.
  • Every box must be completed for the SAAQ to rule; vague or incomplete information delays the decision, and the SAAQ may refuse to pay for the report.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How many psychology hours does the SAAQ cover after the initial assessment?

Balise 1 covers up to 15 treatment hours for any psychological disorder accepted as linked to the accident. Balise 2 can add up to 15 more hours, from 16 to 30, depending on severity and progress, and balise 3, an exceptional measure, covers more than 30 hours for severe and complex disorders. Each step past balise 1 needs the progress summary (4053).

How is the SAAQ initial psychology assessment paid?

The SAAQ's maximum is three hours for doing the assessment and two hours for completing this summary. Bill through the receipt or invoice page (4059 30): Section 4 if the SAAQ pays you directly, or Section 3 as a receipt if the accident victim paid. Treatment hours are claimed separately on the monthly reimbursement request (5696 30).

Who can complete the SAAQ initial psychology assessment summary?

A psychologist or a psychotherapist in private practice, who ticks their professional title and gives their permit number from the Ordre des psychologues du Québec. Parts of Section 3 differ by title: psychologists describe mental state and symptoms and give diagnoses, while psychotherapists describe symptoms and significant psychological difficulties and summarize clinical observations.
