SAAQ 4054: Résumé d'évaluation finale, psychologie

The SAAQ asks for this final summary at the end of a road-accident victim's psychological treatment, for certain files only. The psychologist or psychotherapist reports the problems treated in probable relation to the accident, how symptoms evolved, which therapy goals were met, why follow-up ended and any lasting effects on daily life, work or studies. The SAAQ lists the nine-page package as 4054 34.

Who completes it

The treating psychologist or psychotherapist completes all three sections of form 4054 30, ticks whether the summary was discussed with the accident victim, and signs it. They also complete the receipt, if the victim paid, or the invoice, if the SAAQ pays, on the receipt or invoice page (4059 30).

Submitting and payment

How to submit it
Fax the summary and its receipt or invoice to the SAAQ (the original is not required), or mail them. The PDF can be filled on screen, which carries the person's name and claim number onto the invoice, then printed and signed.
Who pays
The SAAQ pays up to one hour of work for the final summary, through the invoice (Section 4) or as a reimbursement to a victim who paid, using the receipt (Section 3).

Good to know

  • Send it only when the SAAQ asks: its guide reserves the final summary for certain files and says it may refuse to pay for a report that is imprecise, incomplete or not required.
  • Rate effects on daily activities as minor (25% or less of activities or time), moderate (25 to 50%) or severe (over 50%), separating what the person reports from clinical observations.
  • Describe psychological limitations and restrictions for work or studies from clinical data, leave out physical limitations, and say whether factors other than the accident contribute.
  • For psychometric results, name the tests and norms used and give your interpretation rather than raw scores, noting the change if the same questionnaires were used earlier.

Frequently asked questions

When does the SAAQ need a final assessment summary for psychology?

Only when it asks. The SAAQ's guide for psychology forms says the Résumé d'évaluation finale is completed at the SAAQ's request at the end of treatment, where applicable and for certain files only. The initial (4052) and progress (4053) summaries are tied to set points instead: after the initial assessment, and at the 12th or 27th treatment hour.

How is the psychologist paid for the SAAQ final summary?

Through the receipt or invoice page (4059 30) included in the package. The SAAQ pays up to one hour of work for the final summary. Fill Section 4, the invoice, to be paid by the SAAQ, or Section 3, the receipt, if the accident victim paid you and will seek reimbursement, then fax or mail it with the summary.

Is the SAAQ psychology final summary available in English?

No. The SAAQ publishes its psychology forms for private practice in French only. The package includes a one-page guide to the balises (4049 20) and aide-mémoire pages showing what to write in each part of Section 3, including the scale for rating effects on daily activities and definitions of limitations and restrictions.
