Accessible parking permit forms, paratransit and travel medical forms

Forms clinicians sign so patients with disabilities can get a parking permit, ride paratransit, fly or take the train with a support person, or get housing priority.

Patients with disabilities often need a clinician's signature before they can apply for help getting around. Each accessible parking program has its own form, such as Ontario's Application for an Accessible Parking Permit (SR-LV-129E), Alberta's REG3164 and the SAAQ's Demande de vignette de stationnement (form 6375). Paratransit services, including TTC Wheel-Trans and TransLink HandyDART, ask a health professional to confirm how a disability limits use of regular transit.

Who can sign varies. Ontario accepts physicians, NPs, OTs, physiotherapists, chiropractors and chiropodists or podiatrists, while BC's SPARC form is for physicians only. For travel, Air Canada, WestJet, Air Transat and Jazz share the Air Travel Requirements Assessment Form, Porter has its own Forms A to C, and VIA Rail has a support person form. The Easter Seals Access 2 Card and housing priority forms, such as Toronto's 23-0217, are here too. The patient usually pays any completion fee.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can sign an Ontario accessible parking permit application?

A physician, nurse practitioner (Extended Class), occupational therapist, physiotherapist, chiropractor or chiropodist/podiatrist completes Part B of form SR-LV-129E, selecting the qualifying health conditions (A to G) and whether the condition is permanent, subject to change or temporary (up to 12 months). The application is valid for 6 months after the practitioner signs. Permanent renewals, changes and replacements skip Part B, unless a permanent permit expired more than a year ago.

Which airlines use the Air Travel Requirements Assessment Form?

Air Canada, WestJet, Air Transat and Jazz adopted this National Airlines Council of Canada common form in December 2025, and Flair uses it too. A physician, NP or physician assistant completes Section 2. It covers fitness to fly, oxygen or medical equipment onboard, and support person or extra seat requests. Send it to each airline at least 48 hours before departure, since airlines do not share forms with each other.

Who can complete a paratransit application?

Most services accept more than physicians. TTC Wheel-Trans lists physicians, psychiatrists, physiotherapists, OTs, psychologists, RNs and others, as appropriate to the disability. TransLink HandyDART accepts physicians, nurses, OTs, physiotherapists, social workers, optometrists and psychologists, and requires a handwritten signature. Many services may also require an in-person functional assessment, and Calgary Transit Access says a medical recommendation alone does not determine eligibility.

What is the Access 2 Card and who signs it?

Easter Seals Canada's Access 2 Card gives a support person free or reduced admission at participating entertainment, cultural and recreational venues. An accepted health care professional, such as a physician, nurse, psychologist, OT or audiologist, completes Section C. They must have the applicant as a client and must not be related to them. The same form covers renewals and replacements and goes to Easter Seals by email, fax or mail.
