YRT Mobility On-Request Paratransit Application

York Region residents whose disability prevents them from using conventional transit for all or part of a trip apply for Mobility On-Request Paratransit, YRT's door-to-door shared-ride service, with this package. A registered health care professional completes Section B on diagnosis, prognosis, severity, mobility, vision, behaviour and expected duration. YRT staff review complete applications within 14 days.

Who completes it

The applicant or legal guardian completes Sections A, C (when someone else speaks for the applicant) and E, plus optional Sections D and F. A registered health care professional completes Section B, such as a physician, nurse practitioner, registered or registered practical nurse, occupational therapist, physiotherapist, psychologist, optometrist, kinesiologist, speech-language pathologist or social worker.

Submitting and payment

How to submit it
Send the package to Mobility On-Request Paratransit Eligibility by mail, email or fax, or use the drop box at YRT's Richmond Hill office. Section B must be completed and dated within three months of application receipt.

Good to know

  • Eligibility is based on functional ability to use conventional transit, not a medical decision, income or age; conditional riders are travel trained on their first Family of Services trip.
  • Section B asks whether the applicant can independently recognize their destination, tell the driver about a wrong stop and seek help; if not, a mandatory support person is required.
  • Temporary limitations can be marked for 3, 6 or 12 months, and seasonal eligibility is reserved for limitations caused by ice or snow on sidewalks and driveways.
  • For a Support Person Assistance Card, the professional completing Part B of Section F initials the back of the applicant's photo; cards last up to 10 years for those 16 and over.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete Section B of the YRT Mobility On-Request application?

A registered health care professional from YRT's list: licensed optometrist or ophthalmologist, licensed physician, nurse practitioner, physiotherapist, psychologist, registered kinesiologist, registered nurse, registered occupational therapist, registered practical nurse, social worker (MSW or RSW) or speech-language pathologist. Missing information on the certification page stops processing.

Is there a deadline for the health care professional's section?

Yes. Section B must be completed and dated within three months of the application being received. The professional should read the applicant's Section A first, and YRT may contact them to clarify answers, as the applicant authorizes in Section E.

What happens if a YRT paratransit application is denied?

The applicant can call the Contact Centre to arrange an assessment with the Transit Ability Coordinator, and anyone not satisfied with that decision can use the Mobility On-Request appeal process. The coordinator also reviews eligibility regularly and may request an in-person assessment.
