GRT MobilityPLUS Health/Disability Professional Form

Waterloo Region residents applying for MobilityPLUS, Grand River Transit's shared-ride service for people who cannot use conventional transit some or all of the time, need this professional form alongside the Part A application. The professional describes how the disability affects travel on GRT buses and ION trains, without needing a specific diagnosis. Complete applications are reviewed within 14 calendar days.

Who completes it

A health or disability professional able to speak to the applicant's disability and transit ability completes the whole form, for example a physician, nurse practitioner, psychiatrist, psychologist, optometrist, physiotherapist, occupational therapist or social worker. The applicant or their representative completes the separate Part A application.

Submitting and payment

How to submit it
Online submission is encouraged; paper copies can also be dropped off at GRT customer service centres, mailed, or scanned and uploaded with the online Part A application.

Good to know

  • The form asks about functional assessments in the last 24 months, such as TUG or MoCA, with their names, results and a brief interpretation.
  • Separate questions cover physical, cognitive, mental or emotional and vision barriers, safety concerns, sensitivity to crowds or sensory stimulation, and the effect of weather.
  • The support person questions determine eligibility for the Transit Support Person program, which lets a support person ride free; the applicant must provide that person.
  • Managers of pre-approved adult day programs or long-term care centres, and other professionals approved by the Region, may also complete the form.
