Hamilton ATS (DARTS) Application: Long-Term Care

Hamilton's Accessible Transportation Services publishes this version of its application for people living in long-term care homes, whose care needs include 24-hour nursing and personal care, help with daily activities and on-site supervision. It leads to DARTS door-to-door shared rides and Taxi Scrip. A health care professional, including a registered practical nurse, completes Part 2 on function, safety and support needs.

Who completes it

The resident or their representative or substitute decision-maker completes Parts 1A to 1E and the optional Part 3. Parts 2A and 2B are completed by a physician, nurse practitioner, registered nurse, registered practical nurse, chiropractor, physiotherapist, occupational therapist or regulated MSW.

Submitting and payment

How to submit it
Send the completed application to Accessible Transportation Services by mail or fax, or by email at the sender's own risk. Incomplete applications are returned, or ATS follows up for more information.

Good to know

  • Registered practical nurses may complete Part 2 only on this long-term care version; applicants who do not live in long-term care must use the general application.
  • Part 2A asks whether the resident can be safely left unattended at their destination, since DARTS drivers do not unlock doors or buzz apartments.
  • DARTS drivers give no on-board care and do not assist beyond the accessible entrance, so residents who need a support person on DARTS must provide their own.
  • Eligibility can be unconditional, conditional (seasonal from November 1 to April 30, or trip-by-trip) or temporary, and is decided within fourteen calendar days.
