Hamilton ATS (DARTS) Application: General Form

Hamilton residents who cannot use HSR buses because of a physical or functional disability apply here for Accessible Transportation Services, which include DARTS door-to-door shared rides and the Taxi Scrip Program. A health care professional documents functional limitations, prognosis, safety and support needs in Part 2. Eligibility is decided case by case within fourteen calendar days and may be unconditional, conditional or temporary.

Who completes it

The applicant or representative completes Parts 1A to 1G and the optional Part 3. Parts 2A and 2B must be completed by a physician, nurse practitioner, registered nurse, chiropractor, physiotherapist, occupational therapist or regulated MSW, who signs, stamps and gives a registration number.

Submitting and payment

How to submit it
Submit the completed application to Accessible Transportation Services by mail, fax or email. Incomplete applications are returned, and ATS may require a third-party functional assessment.

Good to know

  • Part 2A asks for each limitation's date of onset, staging and prognosis, and whether transportation is needed permanently, temporarily or only in certain seasons.
  • Conditional eligibility can be seasonal, from November 1 to April 30, or trip-by-trip to approved locations, so seasonal limitations should be stated explicitly.
  • DARTS drivers do not unlock doors or buzz apartments, so a passenger who needs that help cannot be left unattended; Part 2A asks about wandering and safety risks.
  • The City warns that outdated application forms may not be accepted, so download the current version from the ATS page before completing it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Who can complete the health care section of Hamilton's ATS application?

Parts 2A and 2B must be completed by a physician, nurse practitioner, registered nurse, chiropractor, physiotherapist, occupational therapist or regulated or licensed Master of Social Work. Registered practical nurses can complete them only on the separate version for long-term care residents.

How long does Hamilton take to decide ATS eligibility?

Up to fourteen calendar days. ATS then issues unconditional, conditional, temporary or not-eligible status. Applicants may be asked to attend a third-party functional assessment, and anyone who disagrees with the decision can file an Eligibility Appeal Form for review by the Eligibility Appeal Panel.

Does the professional say whether a support person is needed?

Yes. Part 2A asks separately whether the applicant needs a support person on HSR buses and on DARTS, always or sometimes, and why: cognitive ability, communication, mobility, vulnerability, behaviour or medical needs. Applicants who need one on DARTS must provide their own.
