TTC Wheel-Trans Eligibility Application: Section C

Torontonians whose disability prevents them from using TTC buses, streetcars or subways for all or part of a trip apply for Wheel-Trans, the TTC's door-to-door specialized transit, with this application. A regulated health care professional completes Section C on the applicant's disabilities, their impact, recent functional tests and support needs. Eligibility can be unconditional, conditional or temporary.

Who completes it

The applicant or representative completes Sections A and B. A regulated professional suited to the disability, such as a physician, psychiatrist, psychologist, registered nurse, occupational therapist, physiotherapist, optometrist, audiologist, chiropractor, speech-language pathologist or MSW, completes Section C and the health care portion of the optional Section D.

Submitting and payment

How to submit it
Send the application by email, fax or mail with Wheel-Trans application in the subject line. Incomplete applications may be returned, and the TTC may request an in-person functional assessment.
Who pays
The applicant covers any costs of completing the application or obtaining additional information.

Good to know

  • If more than one professional is needed, extra copies of Sections B and C are completed and submitted, one set for each professional.
  • The behaviour questions in Section C, such as exiting the vehicle and wandering or threats of harm, help Wheel-Trans decide whether a mandatory support person is required.
  • For a Support Person Assistance Card by mail, the Section C professional signs the back of two colour passport-quality photos; in-person applicants have photos taken.
  • Submitting Section D for a Support Person Assistance Card does not by itself make the applicant eligible for Wheel-Trans service.

Frequently asked questions

Who can complete Section C of the Wheel-Trans application?

A regulated or licensed health care professional suited to the applicant's disability: physician, psychiatrist, physiotherapist, optometrist, audiologist, psychologist, chiropractor, occupational therapist, speech-language pathologist or registered nurse, or a regulated or licensed Master of Social Work. The professional certifies the information and gives an occupation and registration number.

What is the TTC Support Person Assistance Card?

A photo ID card, applied for in optional Section D, that lets a support person travel free on TTC conventional transit or Wheel-Trans with the cardholder. The professional certifies a disability under the Ontario Human Rights Code, permanent or temporary with an expected end date. Processing takes two to four weeks.

Will the applicant need a functional assessment for Wheel-Trans?

Possibly. The TTC may request an in-person functional assessment to learn about the applicant's abilities on the transit system, and the eligibility category is based on the application and any assessment results. Applicants who disagree can request an independent appeal.
