Peel TransHelp Eligibility Application (V-10-048)

Residents of Brampton, Mississauga and Caledon apply for TransHelp, Peel Region's specialized door-to-door transit, with this application. A health care professional completes Section C, describing each disability as permanent, temporary or episodic, its impact on independent travel, recent functional assessments and any need for a support person. Peel decides eligibility within 14 days of a complete application.

Who completes it

The applicant or representative completes Sections A and B. A licensed professional appropriate to the disability, such as a physician, nurse practitioner, psychiatrist, psychologist, physiotherapist, occupational therapist, optometrist, audiologist, chiropractor, speech-language pathologist or regulated social worker, completes Section C; a registered nurse may complete it for long-term care residents.

Submitting and payment

How to submit it
Apply online through Peel's portal, or mail, fax, email or drop off the paper form at the TransHelp office drop box or a Service Peel location. Section C must be completed within 90 days of submitting the application.
Who pays
There is no fee to apply, and the applicant pays any costs of completing the application or obtaining additional information.

Good to know

  • If more than one professional is needed, copy Sections B and C so that each professional completes and signs their own Section C.
  • Section C asks about tests from the past 24 months that measure independent community travel, such as TUG or MoCA, with their purpose and results.
  • Section C also asks whether the disability prevents the applicant from riding in a sedan-like vehicle, and about behaviours such as exiting the vehicle and wandering.
  • Applicants may be asked to attend an in-person functional assessment, and an independent appeal is available if they disagree with the eligibility decision.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete Section C of the TransHelp application?

A regulated or licensed MSW, BSW, RSW or RSSW, or a licensed professional appropriate to the disability: physician, nurse practitioner, psychiatrist, physiotherapist, optometrist, audiologist, psychologist, chiropractor, occupational therapist or speech-language pathologist. Peel's page adds that a registered nurse may complete it for people in long-term care.

How long does TransHelp take to decide eligibility?

Peel Region decides within 14 days of receiving a completed application, including the medical portion. Missing information can cause the form to be sent back, and applicants may also be asked to attend an in-person functional assessment before a decision is made.

Does TransHelp provide a support person?

No. Applicants who need help from another person must bring their own support person, who must be able to meet their care needs during travel and cannot need support themselves. Section C asks whether a support person is needed always, sometimes or never.
