NB Disabled Parking Permit Application (24-01283)

New Brunswick issues disabled parking permits and placards, free of charge, through Service New Brunswick. In Section 2 of this bilingual form, a licensed professional certifies that the applicant cannot travel unassisted more than 50 metres without an assistive device, or has severe respiratory or cardiac symptoms after minimal activity, and whether the condition is temporary or permanent.

Who completes it

The applicant completes and signs Section 1. A licensed physician, occupational therapist, physiotherapist, nurse practitioner or chiropractor completes Section 2, and Service New Brunswick staff complete Section 3 when issuing the permit.

Submitting and payment

How to submit it
The applicant takes the form to any Service New Brunswick centre or mails it to Service New Brunswick.

Good to know

  • For a temporary condition, the professional gives an expected recovery date of no more than five years, the maximum period for which a placard can be issued.
  • Where a wheelchair or lower limb amputation makes the disability visible, Service New Brunswick staff may determine eligibility in Section 3 without medical certification.
  • Companies and organizations that regularly transport people with disabilities can also apply for placards, which may be used only when an eligible passenger is on board.
  • The permit and placard do not exempt the vehicle owner from parking fees, and they must be surrendered when no longer needed.

Frequently asked questions

Is the New Brunswick parking permit form available in French?

Yes. Form 24-01283 is a single bilingual form, titled Demande de permis de stationnement pour personne handicapée in French, with the conditions and the note to licensed health professionals printed in both English and French.
