Fredericton Para Transit Registration (MOB-FRM-060E)

Fredericton residents register for Para Transit, the city's specialized transit service, with this form. Part B is a confidential questionnaire in which a health care provider familiar with the applicant describes physical or cognitive limitations, their severity and duration, mobility devices and assistance needed outside the home. Transit staff use it to decide eligibility and what accommodations can be made.

Who completes it

The applicant or their agent completes Part A and signs the authorization to release information at the start of Part B. A qualified provider familiar with the applicant's condition, such as a physician, nurse practitioner, registered nurse, occupational therapist, physiotherapist, recreational therapist, psychologist or psychiatrist, completes and certifies the rest of Part B.

Submitting and payment

What does the applicant authorize on the form?

The applicant authorizes Fredericton Transit to determine eligibility, to consult the named health care professional or request more information, and the professional to release information to the Transit Division's Operations Supervisor, to be used solely to determine eligibility for Para Transit.
