Air Travel Requirements Assessment Form (NACC)

Since December 2025, Air Canada, Air Transat and WestJet use this common form instead of their own fitness-to-fly forms. Passengers need it when a medical condition may affect safe travel, when they need oxygen or medical equipment on board, or when they need a support person or extra seating. The airline's medical or accessibility desk uses Section 2 to decide on acceptance and accommodations.

Who completes it

The passenger, or a parent, guardian or representative, completes Section 1, including the support person questions and the authorization and consent. The patient's designated doctor, nurse practitioner or physician assistant completes Section 2, initials and dates every page, and signs at the end.

Submitting and payment

How to submit it
Send both sections to the airline's medical or accessibility desk at least 48 hours before departure, though WestJet recommends 10 business days. Once the passenger has signed the consent, the provider may send the completed form directly.
Who pays
The passenger pays any charge for completing the form, as WestJet states on its health page.

Good to know

  • Airlines do not share the form with each other, so the passenger should keep the original to submit to any other participating airline.
  • Section 2 asks for a prognosis for flight, considering cabin pressure equivalent to about 8,000 feet and up to a 30% drop in ambient oxygen partial pressure.
  • Extra seating for obesity applies only to itineraries wholly within Canada and needs height, weight, BMI and a seated surface measurement taken between two marked points.
  • For portable oxygen concentrator users, the form asks for flow rates at rest and on exertion and confirms batteries for at least 1.5 times the flight duration.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete Section 2 of the Air Travel Requirements Assessment Form?

The patient's designated healthcare provider: a doctor, nurse practitioner or physician assistant. You answer all applicable questions, give your licence number and province or state of registration, initial and date each page, and sign the final page. Incomplete forms may be returned, delaying acceptance.

Does the passenger need a new form for each airline?

No. Once completed, the same form can be submitted to Air Canada, Air Transat or WestJet. The airlines do not share it among themselves, so the passenger keeps the original. If the condition or travel details change, the passenger must tell the airline and submit a new form.

Is the form needed for flights to the United States?

For flights to or from the United States, a medical certificate may be required in certain situations, and WestJet accepts a physician's medical certificate dated within 10 days of departure instead of its form. The passenger should confirm with the airline before travel.
