Access 2 Card Application Form (Easter Seals)

The Access 2 Card gives a support person free or discounted admission at participating movie theatres and attractions across Canada when they accompany a person with a permanent disability. New applicants need Section C signed by an accepted health care professional or a disability service provider's executive director. Easter Seals Canada processes applications in about four to six weeks.

Who completes it

The applicant or legal guardian completes Section B and pays the administration fee in Section D. An accepted professional, such as a physician, nurse, psychologist, occupational therapist or audiologist, who has the applicant as a client and is not related to them, completes and signs Section C.

Submitting and payment

How to submit it
Send Sections B, C and D to Easter Seals Canada by mail, fax or scanned email, with the fee paid online first or a cheque or money order enclosed by mail; mailed applications also need a self-addressed stamped envelope.
Who pays
The applicant pays Easter Seals Canada an administration fee for a 3-year or 5-year card, and it must be paid before the application is processed.

Good to know

  • Easter Seals does not accept medical or diagnosis letters in place of Section C; the professional must complete and sign both pages of the section.
  • A photocopy of a CNIB ID card can replace Section C, so applicants who hold one do not need a health professional's signature.
  • Section C is only needed for a first card: renewals of an expired card and lost-card replacements skip it, and online renewals need no form at all.
  • The support person must be 18 or older, and each person with a disability may hold only one Access 2 Card, valid for 3 or 5 years.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What does the health professional certify on the Access 2 Card form?

That the applicant is their client or patient and has a permanent disability requiring a support person to help with communication, mobility, personal or medical needs, or access to goods, services or facilities. Section C also asks for the professional's registration number and a stamp if available.

Can a relative who is a nurse sign Section C?

No. The terms and conditions say the authorizing health care provider must not be related to the applicant, and the applicant must be their client. The executive director of a recognized disability services provider can sign instead of a regulated professional.

How long does the Access 2 Card take?

Easter Seals Canada asks applicants to allow four to six weeks for processing and delivery after a complete application is submitted, and to wait at least four weeks before checking on its status. Paying online is described as the fastest option.
