Porter Form B: Free Seat for a Personal Attendant

Porter passengers aged 12 and older who need a personal attendant in flight use Form B to request a free seat for that attendant on flights wholly within Canada. It applies when care is needed beyond what Porter provides on board, or when Porter's Domestic Tariff requires an attendant for safety. The treating provider explains why an attendant is needed and what duties they will perform.

Who completes it

The passenger, a guardian or a healthcare provider completes Section 1, with functional ability questions and attendant details, and the passenger signs the consent. The treating physician, physician assistant or nurse practitioner completes Section 2.

Submitting and payment

How to submit it
Email or fax the form with reservation details to Porter's Disability Assistance Desk at least 48 hours before departure. Porter makes every reasonable effort for later requests.
Who pays
The passenger pays any fee for completing the form, according to the consent section.

Good to know

  • If approved, the passenger agrees to always travel with a personal attendant on all Porter domestic flights for the duration of the approval, unless Porter advises otherwise.
  • Section 2 asks whether the patient can feed themselves, use the lavatory unaided and take prescription medication without assistance, and why an attendant is needed.
  • The form is not for passengers who prefer a companion for personal reasons or need an attendant only at their destination; international trips or other airlines need a purchased seat.
  • The attendant must be 18 or older, sit beside the passenger outside an emergency exit row, and cannot travel with an infant, a child under 12 or a pet.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What does the doctor confirm on Porter Form B?

Whether the patient has a disability and is fit for air travel, whether they can feed themselves, use the lavatory unaided and take prescription medication without help, and why an attendant is needed, describing the duties the attendant will perform during the flight. You then sign with your registration number.

How long is a Porter attendant approval valid?

One year from the date of the healthcare provider's signature. During that time the passenger agrees to travel with an attendant on all Porter domestic flights, and can name a different attendant for the return trip by contacting the Disability Assistance Desk.

Can a child use Porter's personal attendant seat?

No. The personal attendant service is not available to passengers under 12, and the attendant must be at least 18. The attendant also cannot be travelling with an infant, a child under 12 or a pet in the cabin, or be responsible for another passenger.
