Porter Form A: Additional Seating for Disability

Porter passengers flying within Canada use Form A to request a free specific or additional seat, space permitting, under the Additional Passenger Seat policy of the Accessible Transportation for Persons with Disabilities Regulations. It covers a fused or immobilized leg, which gets an aisle seat on the side of the aircraft that gives the leg extra space, and disability caused by obesity, which gets an adjacent seat. A healthcare provider verifies the need.

Who completes it

The passenger, a guardian or a healthcare provider completes Section 1, and the passenger signs the consent. A physician, physician assistant or nurse practitioner completes Section 2; for a renewal of an approved assessment, a physiotherapist may sign if no comorbidities were identified and fitness to fly is unchanged for two years.

Submitting and payment

How to submit it
Email or fax the completed form with the reservation details to Porter's Disability Assistance Desk as far ahead as possible. Requests received within 48 hours of departure get every reasonable effort.
Who pays
The passenger pays any fee for completing the form, according to the consent section.

Good to know

  • For disability caused by obesity, Section 2 asks for weight, height, BMI and two seated measurements: waist at the umbilicus and maximum hip or buttock width above the gluteal fold.
  • An approved form is valid for one year from the healthcare provider's signature, and the passenger presents it at the airport on the day of travel.
  • The free seat applies only within Canada; Porter cannot guarantee specific or extra seating on flights operated by another airline, including interline connections.
  • When booking within 48 hours of departure, an approved request leads to a full refund of the extra seat, if the refund is requested within 30 days of the first Porter flight.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete Porter Form A?

A physician, physician assistant or nurse practitioner. For a renewal of a previously approved assessment, a physiotherapist may also complete and sign it if the physician identified no other comorbidities in the initial assessment and the patient's fitness for flying has not changed in the last two years.

What does the medical professional confirm on Form A?

The type of disability, a fused or immobilized limb or obesity, with measurements where obesity is the reason, and that the patient can complete the flight safely without extraordinary medical assistance. You sign with your current registration number and province of registration.

Will Porter contact the healthcare provider?

It may. Before approving or declining, Porter may contact the passenger or the healthcare provider to confirm or clarify details on the form, then notifies the passenger by phone or email. Incomplete or illegible forms are not approved, and the Disability Assistance Desk's decisions are final.
