VIA Rail Emotional Support Dog Request Form

VIA Rail lets passengers with a mental health disability recognized in the DSM travel with an emotional support dog at no charge once a request is approved. The form combines the passenger's consent, a medical certificate from the treating professional and the passenger's agreement to VIA's rules for the dog. For a permanent limitation, VIA keeps the approval on file for five years.

Who completes it

The passenger or legal representative completes and signs Section A and agrees to the dog requirements in Section C. The treating psychiatrist, psychologist or attending physician completes the Section B medical certificate.

Submitting and payment

How to submit it
Send the completed request to the VIA Rail Customer Centre by email, fax or mail at least 48 hours before the trip. VIA's accessibility page notes that new requests needing a medical certificate may require up to 96 hours.
Who pays
The passenger is responsible for the cost of the Section B medical certificate.

Good to know

  • For a temporary limitation, the medical certificate must be dated and signed no more than one year before the scheduled travel date.
  • In Section B the professional certifies that the patient needs to travel with an emotional support dog and that they are authorized by their professional association to complete the certificate.
  • The dog must be at least 12 months old, travel in a carrier within VIA's size limits, be muzzled and leashed in stations, and have up-to-date rabies vaccination.
  • The passenger must provide a new valid medical certificate if their health changes significantly, and submit a new form five years after signing for a permanent limitation.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can sign VIA Rail's emotional support dog certificate?

Only the passenger's treating healthcare professional: a psychiatrist, psychologist or attending physician. VIA's instructions add that the patient must have a disability recognized by the Diagnostic and Statistical Manual of Mental Disorders. You provide your licence number and indicate whether the functional limitations are permanent or temporary.

How long does VIA Rail keep an emotional support dog approval?

For a permanent functional limitation, the passenger authorizes VIA Rail to keep the application for five years from the signature date, so new documents are not needed for each trip. After five years, or after a significant change in health, a new form or certificate is required.

Does the emotional support dog travel free on VIA Rail?

Yes. Once VIA validates the completed request, it confirms by email or phone that the dog may accompany the passenger on future trips at no charge. The dog stays in its carrier under the seat or at the passenger's feet, and pit bulls are not accepted.
