Manitoba Accessible Parking Permit Application

Manitoba's accessible parking permits are administered by Manitoba Possible on behalf of the province, and a healthcare provider starts the application rather than the patient. The provider certifies that the person cannot walk unassisted more than 50 metres without great difficulty or danger to health or safety, and chooses a temporary or permanent permit for a vehicle, a motorcycle or both.

Who completes it

The client's details go in Section 1, ideally with an email address for online renewal. Section 2 and the Certification Authority are completed only by a licensed physician, a registered clinic-based nurse practitioner, a chiropractor, an occupational therapist or a physiotherapist.

Submitting and payment

How to submit it
The provider can submit the online GoFormz application in English or French, or the patient mails or brings the paper form to Manitoba Possible. Online applications that include an email address get a payment link by the end of the next business day.
Who pays
The applicant is responsible for all costs of completing the application and pays Manitoba Possible a non-refundable processing fee.

Good to know

  • Temporary permits run 3 to 36 months, for a prognosis expected to change within 36 months, and the provider enters the term required in months.
  • Permanent permits are renewable every 3 years, and Manitoba Possible contacts the holder before expiry; renewal does not require another visit to a healthcare provider.
  • When a temporary permit expires, the patient must visit a healthcare provider again to renew it.
  • Manitoba Possible may ask for medical information at any time to verify eligibility, and the permit can be used in any vehicle as long as the holder is in it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can sign a Manitoba accessible parking permit application?

Only a licensed physician, a registered clinic-based nurse practitioner, a chiropractor, an occupational therapist or a physiotherapist. By signing the Certification Authority, you confirm that the applicant meets the Highway Traffic Act definition of a physically disabled person and that you fully completed Section 2.

Can the patient start the parking permit application?

No. Manitoba Possible says a healthcare professional must apply on the patient's behalf, either through the online GoFormz application or on the paper form. The patient pays the processing fee once Manitoba Possible contacts them, and the permit is then mailed.

Is the Manitoba parking permit form available in French?

Yes. Manitoba Possible posts the paper application in English and French, and the online GoFormz application for healthcare providers is also available in both languages. Both versions cover vehicle and motorcycle permits, temporary or permanent.
