Demande d'admission transport adapté STM (V-2851)

Montrealers apply for STM paratransit with V-2851, the admission form prescribed by Québec's Ministère des Transports for transport adapté across the province. The applicant completes Part 1, and a health or school network professional completes Part 2, the attestation of incapacities. The STM sends a written decision within 45 days of a complete request; temporary limitations such as a broken leg are not eligible.

Who completes it

The applicant, a designated person or legal representative completes Part 1. Part 2 is completed by a professional matched to the diagnosis: an occupational therapist, physiotherapist or physical rehabilitation therapist for motor disabilities, a specialist for staged conditions such as cardiac disease or Parkinson's, or educators, psychoeducators or social workers for intellectual disability or autism.

Submitting and payment

How to submit it
Print, sign and mail the original form with a recent passport-format photo identified on the back and proof of age to the STM's Centre de transport adapté. Incomplete or illegible requests are returned.

Good to know

  • Question 1 asks for a functional classification where relevant, such as NYHA class for cardiac disease, Hoehn and Yahr stage for Parkinson's, or Reisberg stage for Alzheimer's.
  • Question 3 lists seven incapacities, including walking 400 metres on level ground and climbing a 35 cm step with support; communication difficulty alone cannot justify admission.
  • Question 4 records when incapacities occur, such as in winter only, in the evening only, for hemodialysis trips only, or on unfamiliar or complex trips.
  • Question 10 asks whether the information came from an assessment or the file, and the professional stamps, signs and gives a practice number.
