Demande d'admission transport adapté (STAC, Québec)

People in the Québec City area apply for STAC, the RTC's paratransit service, with this French-only admission form based on the provincial transport adapté policy. Part 2, the attestation of incapacities, must be completed by a health or school network professional with access to the diagnosis. A tripartite eligibility committee that meets monthly reviews each request; temporary limitations do not qualify.

Who completes it

The applicant, a designated person or legal representative completes Part 1 and the applicant's annex. Part 2 and the professional annex are completed by a professional matched to the disability, for example an occupational therapist, physiotherapist or physiatrist for motor or organic disabilities, or a physician when the person uses a wheelchair permanently.

Submitting and payment

How to submit it
Send the completed form with a passport-style photo identified on the back and proof of age to STAC by mail or by email. All documents must be included for the file to be processed.

Good to know

  • The six qualifying incapacities cover walking 400 metres on level ground, a 35 cm step, extreme fatigability, orientation in time or space, managing risky situations or behaviours, and communication, which cannot qualify alone.
  • For visual impairment, the committee accepts only visual acuity out of 60 and visual field for each eye written on the form, not attached optometry reports.
  • Only the form and attached supplements are analyzed; STAC will not contact other professionals who did not complete part of the attestation of incapacities.
  • Each professional who completes part of the form signs Question 11 with a practice number, and work done by an intern must be countersigned.

Frequently asked questions

Who can complete Part 2 of the STAC admission form?

A health or school network professional with access to the diagnosis, matched to the disability: occupational therapists, physiotherapists, physiatrists or physical rehabilitation therapists for motor disabilities, plus physicians for permanent wheelchair users; special educators, psychoeducators, psychologists or social workers for intellectual disability; optometrists or orientation and mobility specialists for visual impairment.

Is the STAC admission form available in English?

No. The RTC posts the admission form and its guide for health, school and social service professionals in French only. The guide explains each question in Part 2, the types of companion travel, and the measurements needed for wheelchairs and scooters.

Can someone with a broken leg apply for STAC paratransit?

No. The form states that a temporary limitation, such as a fractured leg, cannot be the subject of an admission request. Applicants must have a significant and persistent disability with mobility limitations that justify paratransit under the provincial admission policy.
