Nova Scotia Accessible Parking Motorcycle Plate Form

Nova Scotians with a permanent mobility disability can get an accessible parking plate for a motorcycle from the Registry of Motor Vehicles with this one-page form. Section 3 accepts either a previously issued accessible parking plate or permanent permit, or a medical certification that the applicant meets the regulatory definition of a permanent mobility disabled person. Motorcycles receive a single rear plate.

Who completes it

The applicant completes the client, vehicle and insurance details and signs the declaration. A physician or licensed nurse practitioner completes the medical certification in Section 3, naming the medical condition; the section also offers the alternative of a previously issued accessible plate or permanent permit.

Submitting and payment

How to submit it
Apply in person at a Registry of Motor Vehicles office or by mail to Service Nova Scotia. Motorcycle plates are always mailed to the applicant.

Good to know

  • The certification covers only permanent disability: limited mobility from paralysis, lower limb amputation, heart or lung disease or another debilitating impairment, as defined in the regulations.
  • Qualifying limits include needing a wheelchair, walker or two of crutch, cane, leg brace or prosthesis, or being limited to 50 metres outdoors by a breathing, cardiopulmonary or neuromuscular condition.
  • Unlike the free accessible parking permit, the motorcycle plate carries a fee paid by the applicant, and proof of insurance is required when plates are issued.
  • Motorcycle plates are valid for one year and renewable, and the Department sends a renewal notice before they expire.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can certify a Nova Scotia accessible motorcycle plate application?

A qualified medical practitioner completes the Section 3 medical certification, and the signature line accepts a doctor or a licensed nurse practitioner. They certify that the applicant is a permanent mobility disabled person as defined and name the medical condition responsible.

What goes in Section 3 of the motorcycle plate form?

Section 3 lists two routes to issuance: a previously issued accessible parking plate or permanent accessible parking permit, or a medical certification by a qualified medical practitioner, who names the medical condition, signs, prints their name and gives a phone number and address.

Can a temporary disability qualify for an accessible motorcycle plate?

No. The motorcycle plate form certifies only a permanent mobility disabled person. Temporary mobility limitations are handled through the temporary accessible parking permit on Nova Scotia's separate permits and plates application, valid for up to six months per certification.
