Toronto RGI Terminally Ill Priority Form (23-0217)

Toronto applicants for rent-geared-to-income housing can request Terminally Ill priority on the centralized waiting list when a physician confirms a terminal diagnosis and a life expectancy of less than two years. The physician also verifies unit needs such as mobility devices, accommodation modifications or an extra bedroom. Access to Housing notifies every applicant of the decision in writing.

Who completes it

The main applicant fills in Section 1 and the patient signs the consent in Section 2. The patient's physician completes Sections 3A to 3C and signs and stamps the authorization in Section 4.

Submitting and payment

How to submit it
The applicant uploads the completed form to their MyAccesstoHousingTO account. For accepted applicants, the waiting-list date reflects when all required documents were received.

Good to know

  • Access to Housing asks physicians to print, use plain language and avoid abbreviations or acronyms, and to include information that supports the prognosis.
  • If the patient needs a separate bedroom for medical equipment or for a caregiver outside the household, a completed Request for Additional Bedroom form must accompany this application.
  • Toronto has no separate urgent medical priority category, so this terminal illness request is the only medical route to priority on the RGI waiting list.
  • Section 3B pairs the diagnosis and supporting documentation with a single choice of life expectancy under or over two years; only under two years qualifies for the priority.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can confirm terminal illness for Toronto housing priority?

A physician who has the patient in their care and has provided the diagnosis. The form asks how long the patient has been under your care, the diagnosis with information supporting the prognosis, and whether life expectancy is less than two years, followed by your signature and stamp.

Does the patient need to consent?

Yes. In Section 2 the patient authorizes the physician to release the requested information to Access to Housing and consents to its use, verification and retention on the housing application. A parent or guardian signs when the patient is under 16.

What happens after the form is uploaded?

Access to Housing reviews the request and notifies all applicants in writing. For accepted applicants, the date on the waiting list reflects the date all required documents were received, including any Request for Additional Bedroom form the physician's answers call for.
