Calgary Transit Access Application (TS 5265)

Calgarians whose disability prevents them from getting to stops, boarding or navigating Calgary Transit buses and CTrains apply for Calgary Transit Access, the city's shared-ride paratransit service, with this form. A treating health professional completes the verification part. Every applicant then has an in-person interview with functional assessments, and the professional's information alone does not decide eligibility.

Who completes it

The applicant or guardian completes Parts A to E, including the consent authorizing release of health information. Part F must be completed by a licensed professional who has treated or directly supported the applicant, such as a physician, nurse practitioner, occupational or physical therapist, psychologist or registered social worker, and never by a friend or relative.

Submitting and payment

How to submit it
Send the full application with Part F, a copy of photo ID and proof of address by scanned email, fax or mail to Calgary Transit Access. Staff then contact the applicant within about 10 business days to book the interview.
Who pays
Any fee for completing Part F is the applicant's responsibility, and Calgary Transit Access does not cover it.

Good to know

  • Eligibility does not depend on age, income, language barriers, fear of transit, trip length or unfamiliarity with the system, only on whether the disability prevents use of buses and CTrains.
  • Part F asks which tasks the applicant can do with their mobility aid, such as crossing a two-lane street, stepping on or off a curb and boarding a CTrain.
  • Calgary Transit Access does not provide attendants, so Part F asks whether a mandatory attendant is needed for behavioural or medical reasons to keep the applicant or others safe.
  • Renewal letters arrive about 30 days before eligibility expires, and renewals usually need no new medical information unless the applicant's ability to use transit may improve.

Frequently asked questions

Who can complete Part F of the Calgary Transit Access application?

A licensed professional who has treated or directly supported the applicant: a physician, surgeon or psychiatrist, an occupational, physical or recreation therapist, a psychologist, registered social worker, registered nurse or nurse practitioner, or a certified orientation and mobility instructor. Friends, acquaintances and relatives cannot complete it.

Does a health professional's form guarantee Calgary Transit Access?

No. Part F states that completing it does not guarantee eligibility. Every applicant attends an in-person interview at a Calgary Transit Access location, where eligibility specialists observe and assess their abilities, and applicants hear within about 5 business days after the interview.

What does Part F ask about the patient's function?

How long you have known the applicant, the primary disability and its onset date, how it prevents transit use, any balance or gait concerns, and how day-to-day function is affected in functional health, sensory, cognitive, mental health and behavioural areas. Seizure disorders may also need a separate Seizure Supplement Form.
