NL Accessible Parking Permit Application (Blue Zone)

Newfoundland and Labrador residents apply to the Motor Registration Division for a Blue Zone accessible parking permit with this one-page form. A physician or nurse practitioner certifies in Part 2 that the applicant meets at least one of four criteria, covering lower-limb mobility, lung disease, cardiac disease or a visual or other impairment, and marks the permit permanent or temporary.

Who completes it

The applicant or guardian completes and signs Part 1. A medical practitioner or nurse practitioner completes and signs Part 2 the first time the person applies.

Submitting and payment

How to submit it
Send the form to the Motor Registration Division by email or fax, or by mail or drop box at any Motor Registration office. Processing takes 5 to 10 business days.

Good to know

  • The lung criterion is objective: FEV1 under one litre on spirometry, or arterial oxygen tension under 60 mm Hg on room air at rest.
  • The cardiac criterion requires Canadian Heart Association functional Class III or IV, or the need for a wheelchair or special transit facility.
  • Temporary permits last six months and cannot be renewed, so a new application is needed; permanent permits last five years and renew without a new medical form.
  • The form reminds clinicians of their duty under section 174.1(1) of the Highway Traffic Act to report drivers whose medical condition could affect safe driving.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can sign the NL accessible parking permit application?

A medical practitioner or nurse practitioner completes Part 2 on a first application, certifying that the applicant meets one or more of the four criteria and marking the permit permanent or temporary. Renewals of permanent permits do not need a doctor or nurse practitioner.

How long is an NL accessible parking permit valid?

Temporary permits are valid for six months and are not eligible for renewal, so a new application is needed. Permanent permits are valid for five years and can be renewed online through MyGovNL or with a paper renewal form, without a new medical certification.

Is there a fee for the Blue Zone parking permit?

No. The Motor Registration Division charges no fee for the permit. It is mailed or held for pick-up after 5 to 10 business days, and can be used in any vehicle in which the permit holder is a passenger.
