London Transit Specialized Transit Application

Londoners whose permanent or temporary disability prevents them from using the city's fixed-route buses apply for Specialized Transit, London Transit's paratransit service, with this five-page form. The applicant describes their travel needs and abilities, and a health care professional reviews those answers before certifying the disability's effect in Part D. Eligibility can be unconditional, temporary or conditional.

Who completes it

The applicant or a designate completes Parts B and C and signs the declaration. One of the listed professionals, such as a licensed physician, nurse practitioner, registered nurse, occupational therapist, physiotherapist, psychologist, psychiatrist or optometrist, reviews Parts B and C and completes Part D.

Submitting and payment

How to submit it
Mail or drop off the completed application, with all parts, at the London Transit Commission office, attention Specialized Transit Services.

Good to know

  • Part D asks whether the condition prevents fixed-route use at all times, in winter only, or unless an attendant is present, and how long service will be needed.
  • All London Transit buses are accessible, and drivers help with priority seating and securing mobility aids, so Part D asks whether the applicant could ride with that help.
  • London Transit may request more information, an interview by phone or in person, or a functional assessment, and denied applicants have the right to appeal.
  • Eligibility does not depend on age, income or the trip purposes listed in Part B, which are used only for service planning.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete Part D of the London Specialized Transit application?

A licensed physician, licensed physiotherapist, licensed optometrist, ophthalmologist or eye physician, registered occupational therapist, certified psychologist or psychiatrist, or a registered nurse practitioner or registered nurse, as appropriate to the case. You review Part A and the applicant's answers in Parts B and C first.

Does London Transit contact the professional who completed Part D?

It may. The applicant's declaration consents to the Commission's health care authority discussing the application and eligibility with the professional who completed Part D. London Transit's health care professional also advises whether an interview or functional assessment is needed.

What happens if the applicant needs a personal care attendant?

Part C asks whether a personal care attendant must travel every time. If the answer is yes, London Transit provides service only when the attendant is travelling with the customer. London Transit's page notes that a support person can travel free once eligibility is confirmed.
